Objective:To observe serum ferritin expression levels in patients with obstructive sleep apnea hypopnea syndrome (OSAS), and to analyze the relationship between OSAS-related diabetes and serum ferritin.Then do exploration about the the possible pathogenesis of abnormal glucose metabolism and type2diabetes caused by OSAS.Methods:A case-control study. To select51male patients with OSAS with PSG in sleep detection center,diagnosed by polysomnography (PSG) from February2011to September2011. The diagnostic criteria bases on "obstructive sleep apnea-hypopnea syndrome treatment guidelines". One side,OSAS with diabetes mellitus (OSAS with DM) group are15cases; Other side,36cases of patients with OSAS alone. They were divided into mild group14patients,moderate and severe group28cases,based on the apnea-hypopnea index(AHI).To select randomly18health male adult in examination center as normal control from July2011to September2011;To collect type2diabetes (T2DM)14cases of male patients in the endocrine department during2011. All subjects write the same sleep questionnaire and to carry out polysomnogram monitoring all night for7hours at least,and analyze the respiratory disturbance index (AHI)and nighttime minimum oxygen saturation (SaO2min); Collecting empty stomach peripheral venous blood in two test tube at7am on next day.To examin the plasma glucose (FPG), glycosylated hemoglobin level (HbA1C), fasting insulin levels (FIN), and serum ferritin level (SF) by ELISA; All data were analyzed statistically.Results:1.The SF levels of mild OSAS group, moderate and severe OSAS group,OSAS with DM group and T2DM group, respectively (266.8±85.1) ng/ml,(355.3±92.3) ng/ml,(401.7±78.2) ng/ml,and (294.3±57.4) ng/ml] were higher than control group (196.8±65.4) ng/ml,the differences were statistically significant (all P<0.01).,and OSAS group and OSAS with DM group.mild OSAS group and severe group were significant difference (P<0.01).2. The levels of HOMR-IR in moderate and severe OSAS group, OSAS with DM group and T2DM group [(5.95±1.22),(7.43±1.87),(6.01±2.63)] were higher than those in control group (1.61±0.88) and mild OSAS group (2.68±0.99), the differences were statistically significant (all P<0.01), OSAS with DM group,moderate and severe OSAS group, and the T2DM group were significant difference (all P<0.01);3.SF correlated with AHI, LA, SLT90and SaO2min(r=0.74,0.53,0.48,-0.55, all P<0.01);and correlated with FPG and HOMR-IR(r=0.34,0.54,all P<0.01). After controlling the HbA1C, FPG and HOMR-IR,the SF was still correlated with the AHI, SaO2min, LA (s),SLT90,the differences were significant (r=0.82,-0.64,0.71,0.42, P<0.01).Conclusion:(1) Compared with normal control group,serum ferritin levels in OSAS patients were significantly increased;(2) Compared with the moderate and severe OSAS patients, serum ferritin levels in OSAS with diabetes were increased more significantly;(3) Serum ferritin may play a role in the occurrence and development of abnormal gluose metabolism and type2diabetes in OSAS.
