Objects1. To evaluate the clinical effect of compound glabrous greenbrier rhizome granules on preventing and curing acute gouty arthritis, by observing the changes of uric acid level and the frequency of gouty arthritis2. To explore the mechanism of compound glabrous greenbrier rhizome granu-les on preventing and curing acute gouty arthritis by analyzing its effc-tion on the level of IL-1β IL-6TNF-α of synovial membrane homogenate.3. To develop a novel rat model of acute gouty ankle arthritis by monosodium urate(MSU) crystsl-induced and use it to assess the effects of compound glabrous greenbrier rhizome granules on sedimentin.Methods1.43patients of gouty arthritis were randomly divided into compound glabrous greenbrier rhizome granules group and Allopurinol group.The2group acceptted different treatment scheme. At the beginning of the20day. patients should take medicine every day.After20days, patients in compound glabrous greenbrier rhizome granules group must obey the treatment scheme that take the medicine7day every7days. The clinical symptoms and laboratory data before and after treatment were noted and compared.2.54SD male rat of SPF grade were randomly divided into High-dose, Medium-dose, Low-dose compound glabrous greenbrier rhizome granules group, Colchicine group, Model group and Control group, the corresponding drugs were administrated7days. On the8th day, MSU crystals were injected into the rat joint,6hours latter, its blood was obtained to detect sedimentin and leucocyte. Simultaneously, synovial membrane of knee joint were removed to detect the level of IL-1β IL-6TNF-α by Elisa and observe structure change of the rat.Results1. The significant decrease of the degree of uric acid was found in the three groups of gouty arthirtis patients after treatment (P<0.05).2. The total effective rate was89.47%in compound glabrous greenbrier rhizome granules,76.19%in Colchicine group, There was no statistically signi-ficant difference between glabrous greenbrier rhizome granules and Colchicine group (P>0.05).3. Detected by ELISA, the IL-1β IL-6TNF-α in the synovial membrane homogenate were decreased significantly in High-, Medium-, Low-dose compound glabrous greenbrier rhizome granules group han that in Model group (P<0.05),The effects of High-and Medium-dose compound glabrous greenbrier rhizome granules group were similar with Colchicine group (P>0.05).4. Imposed by different factors to each group, oral administration of compound glabrous greenbrier rhizome granules could significantly reduce the sedimentin in rat, and it was significant difference than that in Model group (P<0.05). The effects of High-dose compound glabrous greenbrier rhizome granules decoction group were similar with Colchicine group (P>0.05). the leucocyt in the blood were decreased significantly in Colchicine group than that in Medium-, Low-dose compound glabrous greenbrier rhizome granules group and Model group (P<0.05).Conclusions1. Compound glabrous greenbrier rhizome granules can reduce the frequency of acute gouty arthritis and the uric acid, while it is safety.2. Compound glabrous greenbrier rhizome granules can reduce the sedimentin effectively in acute gouty arthritis rat, Its mechanism of preventing and cruing gouty arthritis might relate with inhibition to the IL-1β、IL-6expressions.
