The Application Evaluation Of Chinese Menopause Rating Scale

ObjectiveMenopause is a necessary stage for each female, which along with the decline of ovarian function leads to the level of endocrine change significantly. With the arrival of the aging society and increasing pressure in modern social, the patients with menopause syndrome also increased year by year. The patients of this period bear the important responsibility of family and society, their health directly affects the family and society. Traditional Chinese medicine has exactly effect on treating menopausal syndrome in the clinical practice, but this effect has not been approval by modern medicine, one of the reasons is lack a TCM therapeutic evaluation index which capable of being recognized. Chinese Menopause Rating Scale (CMRS) is an evaluation scale for evaluating menopausal syndrome of traditional Chinese medicine curative effect which developed in strict accordance with the scale development process, after three test, experiment, a formal investigation and adjustment, the CMRS contains35entries now. During "fifteen", the CMRS(Formal investigation Version40items) has been proven had well reliability, validity, response degree. Menopause syndrome is a typical psychosomatic disease, and CMRS is a scale to evaluate psychosomatic symptoms, this study based on the national Ministry of science and technology" eleven five" supporting project, evaluate the CMRS.MethodA national multicenter, randomized, controlled study had been designed, in August2009to December2010, patients came from6hospital of Guangzhou, Tianjin, Shanghai, Wenzhou, Chengdu, Hubei. Patients met the diagnostic criteria were374cases,211cases of them were eligible subjects. Patients were treated by TCM psychosomatic treatment or Livial with supportive psychotherapy. The course was8weeks. The patients filled out CMRS/Kupperman Index, Self-rating Depression scale and Self-rating anxiety scale before and after treatment, evaluating the reliability, validity, response degree analysis of CMRS, and with the KI scale, self-rating depression scale, self-rating anxiety scale as the criterion mark.ResultCronbach a coefficients of CMRS and each dimension were higher than0.7, except for the social dimension, the CMRS and somatic, psychological dimension split half reliability were higher than0.7, this suggest that CMRS had good internal consistency.Validity evaluation used structural validity, criterion-related validity. Construct validity was evaluated by factor analysis, all entries of CMRS were extracted9common factors, the entries of body dimensions were extracted5common factors, social dimension were only extracted1factor, these factors entry categorizes better, suggested the CMRS had good construct validity. Entries of psychological dimension were extracted to2or3factors, but the entry categorizes slightly.12factors of CMRS were extracted3common factors, consistent with the original dimensions of. The results of correlation analysis showed that CMRS total score and each dimension respectively have positive correlation, the total score had high correlation with three dimension, the correlation coefficient was low between physical dimensions and psychological dimension/social dimension, the psychological dimension and social dimension had high correlation. Each factor and the corresponding dimension had positive relationship, correlation coefficient were high except disease characteristics factor. Overall, CMRS has good construct validity. With KI, SDS, SAS as a criterion, evaluation criterion-related validity of CMRS, the results showed that:the CMRS total scores and somatic dimension scores had high correlation coefficient with KI; total score, psychological dimension score," depression" factor had high correlation with SDS. The correlation coefficient of CMRS and SAS was poor.The evaluation of reaction degree used paired t-test with the score before and after treatment, the results showed that, all entries, dimensions and factors, total score had good reaction degree.ConclusionCMRS has good reliability, validity, responsiveness, can be used to the menopause syndrome efficacy evaluation.
