Clinical Observation Of Sepsis Intervened By Tanreqing Injection In The Early Stage

ObjectiveSepsis had a complex etiology and pathogenesis, and had a close relationship with multiply systems and organs’pathophysiology alteration. Treatment methods of sepsis were non-specific, supportive treatment for multiple systems and organs’injuries caused by sepsis was main. In recent years, according to sepsis pathogenesises and researches of evidencebased medicine, the targeted therapies were used in clinical treatment, and had got some curative effects. TCM treatment features were antibacterium/antiviral effect, anti-inflammatory effect, regulating immunity, low drug resistance rate, fewer side-effects, etc. for sepsis. Integrating traditional Chinese and western medicine treatments were often used at present, and had got some curative effects. So the further study of effects and clinical efficacy treated by TCM treatment on sepsis has some important clinical significance.The septic patients have a clinic intervention with adding the heat-clearing and detoxicating drug-Chinese herba preparation-Tanreqing injection to the western medicine conventional treatment in this study, according to the features of septic etiology and pathogenesis, combined with the modern pharmacology studies of TCM, discuss the effects of blocking-up inflammation, improving hyperlactacidemia caused by hypoxia and hypoperfusion, development of disease course and prognostic in the septic patients treated with the heat-clearing and detoxicating drugs, and evaluate the curative effects by treatment which combines TCM with western medicine preliminarily, offer evidences and thinking for treating sepsis by TCM adjuvant therapy.MethodsThe objective is26patients correspond with the septic diagnosis standards of traditional Chinese and western medicine in ICU, pneumology department, department of nephropathy, General internal department and other related departments of Guangzhou University of TCM. It is a simple, random and control study, and the patients were divided into two groups, the control group and the observation group:the first group was treated with conventional western medicine method, and the other one used the heat-clearing and detoxicating drug-Chinese herba preparation-Tanreqing injection based on conventional western medicine method, then APACHEII scale scores were estimated, the Lac, PCT and germiculture in peripheral blood was tested, tongue and pulse manifestations were observed, cold or hot conditions were realized, sweating situations were surveyed, defecate and urinate conditions were investigated, realized if it appeared new organs failure, and the fatality rate in28days is analysed. Analyse datas with SPSS17.0statistical software, measurement datas are shown by mean+standard deviation (x±S), contrast by t test between means; Enumeration datas, rates are processed by X2test; Correlation analysis is managed by Pearson correlation analysis. Difference expresses statistical significance when P<0.05, and extremely when P<0.01.Results1. The Lac and PCT level in peripheral blood of both the observation and control group after treatment is lower than before treatment(P<0.01), and in the observation group reduces significantly compared with in the control group(P<0.01);2. The APACHEII scale scores of both the observation and control group after treatment are lower than before treatment(P<0.01), and in the observation group reduce significantly compared with in the control group (P<0.01);3. The number of deaths and the fatality rate in28days in the observation group has a slightly downward trend compared with in the control group, but the difference has no statistics significance(P>0.05);4. The APACHEII scale scores were related to both the PCT and Lac levels in peripheral blood, and it is the positive correlation, r=0.742/0.785.Conclusion1. Compared with the conventional western medicine method, adding Tanreqing injection to conventional western medicine method can cut down the level of the inflammatory mediator-PCT in peripheral blood, maybe Tanreqing injection plays a role in restraining the inflammatory response.2. Compared with the conventional western medicine method, adding Tanreqing injection to conventional western medicine method can cut down the level of the anaerobic glycolysis production-Lac in peripheral blood, maybe Tanreqing injection can improve hyperlactacidemia caused by hypoxia and hypoperfusion for sepsis patients.3. Compared with the conventional western medicine method, adding Tanreqing injection to conventional western medicine method can make the APACHEII scale scores lower, maybe Tanreqing injection can improve the sepsis patients’conditions and prognosis in some way.4. The APACHEII scale scores have the positive correlation to both the Lac and PCT in peripheral blood, maybe they can reflect illness severity to a certain extent, so it is more significant to judge septic patients’ conditions and prognosises if using the APACHEII scale scores with the Lac and PCT in peripheral blood.
