ObjectiveThe study aims to explore the management model for rural public health services, and analyzes the effectiveness of public health work run under the management of this model to evaluate the feasibility and effectiveness of the management model as well as to provide practical experience of public health management for the vast rural community all over China, through in-depth analysis of the public health project management model in Jiangshan Town, Yinzhou District,Methods1. Literature searchTo search the related literature of public health service management at home and abroad in order to construct the theoretical basis of the community public health project management in Jiangshan county.2. Status surveyTo survey the status before and after the implementation about the project of public health programs; and to compare the level of public health services with the similar economic level areas. 3. Questionnaire survey methodTo survey the public health job satisfaction of residents, questionnaire is to be designed by literature research, expert advice, and is to be improved through the pre-investigating, the questionnaires is surveyed using question and answer form.4. in-depth individual interviewsTo interview three categories of persons including three Health Bureau leaders, three central leaders and five village leaders, to understand their views of community health service centers and the management of public health programs.5. Data entry and statistical analysisThe epidata3.1software is applied for data entry, and the spss13.0software is used for statistical analysis, baseline data is analyzed by descriptive statistical methods, the difference before and after the implementation is compared by chi-square test or t-test methods.Results1. Public health project management model has been established, the organization and management structure has been built and the project list has been prepared according to the project management theory, and the project management is operated in accordance with the project life cycle.2. It is a successful practice of public health professionals in team building. Each health service station is equipped with a full-time staff of public health services, the funds guaranteed by the financial security is required its responsibility to the people. After the implementation, the quality of public health personnel has improved significantly.3. The health status of residents has improved. Through the implementation of the project management of patients with major chronic diseases including hypertension and diabetes, monitoring indicators such as blood pressure and blood glucose are significantly decreased. The level of understanding of the related diseases has improved significantly, including hypertension awareness from68.37%to97.66%, and awareness of diabetes from70.58%to95.59%.4. The scope and capacity of public health services has improved significantly. After the implementation, the range of the public health services has extended to three categories twelve items from the original basis. Public health services have been greatly enhanced. Resident health file rate, blood pressure and diabetes filing rate have increased significantly. The detection rate, manage rate, and standardize management rates, treatment rates and control rates of hypertension were15.8%,92.1%,89.0%,84.5%and75.1%respectively, the detection rate, management rate, regulate rates, treatment rates and control rates of diabetes were3.3%,97.9%,95.4%,67.9%and68.7%respectively, in excess of the average levels of yingzhou district and the target rate of chronic disease management assessment in Zhejiang Province, and superior to the management indicators of the level of the similar economic development areas such as Jiangsu, Fujian. Follow-up people increased significantly, and the follow-up model has gradually transformed from the collective follow-up before the implementation to a family home follow-up. With the public health services carried out, the development of basic medical services is promoted at the same time.5. The information technology platform is successfully built and the sharing of resources is realized. Jiangshan Community has integrated Hospital Information Management System (HIS), inspection and management information system (LIS), chronic disease management, and women and children’s health care system into the Comprehensive working platform of community physicians, so that the public health commissioner can make real-time records of follow-up objects of various information under the help of3G General computer information services.6. The management of public health programs have won high praise from the Health Bureau leaders, the village committee leaders and residents. Health Bureau leaders and the village leaders all hold positive attitudes to public health work, and the satisfaction rate of chronic disease management residents have reached97.4%. In general, the cultural quality and service capabilities of the Commissioner of Public Health has been gained a high degree of evaluation and accreditation of local people.7. The major shortcomings:1) The temporary and rehired staff in the Commissioner of Public Health team are in the majority, being unstable phenomenon; and there are some rural doctors in the team structure, with weak professional base and needed to strengthen training.2) The public health funds guarantee mechanism remains to be further improved, and being lack of public health funding for Special Appropriations on temporary employment and rural doctors; the public health budget is sufficiently sophisticated, it is urgent to establish the mechanisms for implementation of budget management.3) Information construction has not yet achieved resource sharing among the higher level hospitals and other parts of the networking.ConclusionsThe Jiangshan community public health project management model has been basically established, and well-functioned in rural communities.It is a successful practice of public health professionals team building, as well as an innovation of domestic public health personnel management modelThe establishment of information platform and its resources sharing have effectively promoted the work of public health, and have made public health management performance improved significantly.Obvious social benefits after project management implementation: the range and levels of public health services has been improved, higher than the regional average and more than the levels of similar areas.
