Objective In this study, we assess the factors for prognosis by analyzing the clinical data of patients with severe aneurysmal subarachnoid hemorrhage (aSAH), which can provide the experience of prophylactic management. Methods Retrospectively analyze the clinical materials of66patients with WFNS Ⅳ-Ⅴ grade aSAH who were admitted to Second Affiliated Hospital Zhejiang University School of Medicine from September,2010to September,2011. The prognosis was measured by the modify Rankin scale (mRS) grade when discharged from hospital. Results Among66patients,36had good prognosis (mRS0-3) and30had bad prognosis (mRS4-6). Single factor analysis demonstrated that age, WFNS grade, Claassen grade, operation to decrease intracranial pressure, hypertension on mRS grade at discharge were significantly different between two groups (P<0.05or0.01). And gender, location and size of responsible aneurysm, timing of operation, and treatment method of aneurysm (clipping or coil embolism) were not significantly different between the groups (P>0.05). Furthermore, multiple factor analysis revealed age, WFNS grade, Claassen grade, operation to decrease intracranial pressure, hypertension were respectively independent factor (P<0.05). Conclusion The prognosis of severe aSAH may be affected by factors such as WFNS grade, Claassen grade, operation to decrease intracranial pressure and history of hypertension. And gender, location and size of responsible aneurysm, timing of operation, and treatment method of aneurysm (clipping or coil embolism) were not significantly different between the groups.
