Objective To investigate the association between three observation index: carotidartery itima-media thickness of (IMT), high sensitivity C-reactive protein(hs-CRP)and N-terminal pro-brain natriuretic peptide(NT-proBNP) and the acute coronarysyndrome(ACS) patients complicated with type2diabetes by observing the change ofthree observation index in the above-mentioned patients and the change relationship withprognosis in these patiens given to standard treatment.Method The study population included230patients with ACS,aged62±12diagnosed in our hospital from January2010to June2011.60cases who werecomplicated with type2diabetes and had course of desease beyond3years wereselected as ACS complicated with type2diabetes group.60cases who had no diabetesand had normal glucose tolerane were selected as ACS without diabetes group.60healthy people who were examined to have no hypertention,hyperlipemia anddiabetes in our hospital were chosen as healthy control.60cases who had course oftype2diabetes beyond3years and had no change in electrocardiogram wereselected as diabetes control. Carotid artery IMT was determined by color Dopplerultrasonography. Serum hs-CRP NT-pro BNP levels were measured withfluoroimmuno assay.Result1.General observation index of the ACS patients such as triglyceride, low densitylipoprotein increased significantly.2. Carotid artery IMT,hs-CRP,NT-proBNP in ACS patients increased significantly inthe level. IMT and hs-CRP level of ACS patients with diabetes significantlyincreased than that of non-diabetic ACS patients, the difference was significant.3. Carotid artery IMT, hs-CRP and NT-proBNP in ACS patients were positivelycorrelated.4.Setting a higher degree of positive index, these three observation index have susceptibility higherly on the diagnosis of ACS complicated with diabetes;setting alower standard as a positive index, the specificity decreased significantly, while theJoint use of two or three index,a certain extent, improve the specificity of diagnosis.5. hs-CRP and NT-proBNP of ACS diabetes mellitus extend the downward trend withthe treatment time after standardized treatment show time-effect relationship,whilecarotid artery IMT was no significant change.6follow-up one and a half years, these index level in cases with cardiovascular eventsare generally higher than those withoutt cardiovascular events, especially in cases ofserious cardiovascular events (such as suden death and heart failure), the level ofhs-CRP and NT-proBNP increased obviously.Conclusion1.Carotid artery IMT,hs-CRP and NT-proBNP in ACS patients increased significantlyin the level. IMT and hs-CRP level of ACS patients with diabetes significantlyincreased than that of non-diabetic ACS patients.The result has a certain clinic valueon the early diagnosis and prevention of ACS to diabetes patients.2.When Carotid artery IMT,hs-CRP and NT-proBNP are set a lower standard as apositive index, the Joint use of two or three index,a certain extent, improve thespecificity of diagnosis in ACS patients with diabetes3.The carotid artery IMT, hs-CRP, NT-proBNP in cardiovascular events cases aresignificantly higher than the group that did not happen, that have certain clinicalvalue.on treatment and prognosis danger evaluation.
