Objective To analyze the clinical features of peripheral T-cell lymphoma, not otherwise specified, in order to provide reasonable evidences to stratify patients with PTCL-NOS for early diagnosis and risk-adapted therapies.Methods The retrospective study analyzed the clinical and immunohistochemical features of104cases of PTCL-NOS which were diagnosed and classified according to the2008World Health Organization (WHO) from the first affiliated hospital of Guangxi Medical University from2002to2010.Results①This study had a median age of42years onset in104cases of PTCL-NOS. The ratio between male and female was1.7:1.The Han nation is the most common in this disease (n=78), the Zhuang followed (n=25) and finally the other minorities (n=1). The most predilection sites of PTCL-NOS was the superficial lymph nodes(27.9%), parallel by waldeyer ring (23.1%)and nasal cavity and paranasal sinuses(23.1%). Pure nodal disease only present in14.4%, nodal and extranodal disease in45..2%, and extranodal disease present in40.4%of the patients.②By Ann Arbor staging, in all104patients,stage I was present in4.8%, stage Ⅱ36.5%, stage Ⅲ17.3%, stage IV41.3%. According to the IPI/alPI prognostic index, there was only32.7%of all patients present in low-risk group, low intermediate21.2%, and the advanced stage (Ⅲ/Ⅳ) disease46.2%. If followed by the PIT prognosis index, fewer of patients (25%) had advanced stage (Ⅲ/Ⅳ) disease comparing to IPI/aIPI prognostic index.③A total of six cases(5.8%) had BMI in this study. All were divided into group B. And general cases present lymphadenectasis. At least one of abnormalities in the peripheral blood of three lines, such as abnormalities of Wbc, anemia or thrombocytopenia.④The vast majority of cases expressed T-cell associated antigen such as CD2, CD3, CD7, CD43, CD45Ro, and a small expressed the B-cell associated antigen like the CD20and CD79. It also expressed CD30, but did not express CD56, TdT and ALK antigen.⑤Abnormal levels of serum lactate dehydrogenase (LDH) increased in this group accounted for37.5%.The levels of serum LDH in stage Ⅲ/Ⅳ patients is higher than that in stage Ⅰ/Ⅱ. The difference of serum LDH levels present statistically significant (p=0.012). The level of serum LDH between the group B and group A also prensent statistically significance(p=0.001).Conclusion1) PTCL-NOS is more common in male patients, especially in the middle age people.2) Extranodal disease is more frequent than nodal PTCL-NOS.3) PTCL-NOS could express all T-cell associated antigens, but it often loss one or more antigen of them. The aggressive disease also could express antigens of B-NHL and some special types of lymphoma. It expressed infrequently antigens of NK/T cells lymphoma, ALCL, and precursor cell lymphoma.4) The level of serum LDH at stage Ⅲ/Ⅳ was higher than stage Ⅰ/Ⅱ, also LDH level in group B was higher than group A.
