Objective The first purpose of researches was to explore the relationship of SUVmax and relevant factors in small peripheral lung adenocarcinoma; The second purpose was the comparison analysis of the18F-FDG PET/CT imaging of the peripheral lung adenocarcinoma and squamous cell carcinoma. The other purpose was to evaluate the18F-FDG PET/CT value in the diagnosis of staging of peripheral lung cancer.Methods The researches were in four parts.The first part was to stuy the statistical analysis and correlation analysis of peripheral lung adenocarcinoma (confirmed by pathology less than3cm)about differentiation degree, the biggest diameter of the original tumor and SUVmax. The second part was to stuy the statistical analysis and correlation analysis of peripheral lung adenocarcinoma and squamous cell carcinoma about differentiation degree, the biggest diameter of the original tumor,the clinical stage and SUVmax. The third part was to stuy the value of18F-FDG PET/CT in the diagnosis of small peripheral lung cancer(The biggest diameter was less than2cm). The fourth part was to stuy the value of18F-FDG PET/CT in the TNM staging of small peripheral lung cancer(The biggest diameter was less than3cm).Result1、(1)The SUVmax of high differentiation adenocarcinoma was significantly lower than the low differentiation adenocarcinoma;The SUVmax of moderately differentiated adenocarcinoma had no obvious difference between high and low differentiation adenocarcinoma.(2)The SUVmax of adenocarcinoma with the maximum diameter more than2cm was significantly more than that of the maximum diameter less than2cm.(3)The differentiation degree and maximum diameter of adenocarcinoma were positively correlated with SUVmax.2(1)The SUVmax of squamous cell carcinoma was significantly more than that of adenocarcinoma.(2)The SUVmax of low differentiation adenocarcinoma,moderately differentiated of poorly differentiated squamous cell carcinoma were higher than that of high differentiation adenocarcinoma.(3)The SUVmax of adenocarcinoma Ⅲ+Ⅳ stage were significantly more than that of Ⅰ+Ⅱ and the stage of squamous cell carcinoma had no obvious difference.(4)The SUVmax of adenocarcinoma was positively correlated with maximal diameter and TNM staging. The SUVmax of squamous cell carcinoma was positively correlated with maximal diameter.3、The accuracy of18F-FDG PET/CT diagnostic was84.4%,accordingly the sensitivity was91.4%, specificity was60%, the positive predictive value was88.9%, the negative predictive value was66.7%:The malignant tumors in CT and the high metabolic in PET can diagnose malignant tumors;The benign pathological changes in CT and the low metabolism or no metabolic in PET can diagnose benign pathological changes; The benign pathological changes in CT and the high metabolism in PET had high possibility in the diagnosis of benign pathological changes; The malignant tumors in CT and the low metabolism or no metabolic in PET had high possibility in the diagnosis of malignant tumors.3、18F-FDG PET/CT had more accuracy than a separate CT in in peripheral small lung cancer T, N staging. For M staging,PET/CT can find more lesions.Conclusion1、(1)The SUVmax of low differentiation adenocarcinoma was higher than that in well-differentiated group;The SUVmax of moderately differentiated adenocarcinoma had no obvious difference between high and low differentiation adenocarcinoma; The lower the Adenocarcinoma differentiation,the greater the SUVmax.(2)The SUVmax of adenocarcinoma with the maximum diameter more than2cm was significantly more than that of the maximum diameter less than2cm;The more the the maximum diameter,the greater the SUVmax.2、The SUVmax of low differentiation adenocarcinoma,moderately differentiated of poorly differentiated squamous cell carcinoma were higher than that of high differentiation adenocarcinoma;The more the the maximum diameter of Adenocarcinoma and squamous cell carcinoma,the greater the SUVmax.3、To less than2cm in diameter of small peripheral lung cancer,the accuracy, sensitivity and specificity of18F-FDG PET/CT were higher than that of simple CT and simple PET4、The accuracy of18F-FDG PET/CT in small peripheral lung cancer staging in TNM staging was more than CT.For the correct clinical staging and treatment,18F-FDG PET/CT has a significant guiding role.
