[Objective]:To investigate the clinical characteristics, treatment and prognosticfactors of neoadjuvant chemotherapy in locally advanced gastric cancer.[Methods]:The clinical data of70patients histologically diagnosed with locallyadvanced gastric cancer in Fu Jian Provincial Hospital and treated with NAC andsurgery from January2006to June2011were analyzed retrospectively.[Results]:Pathologic complete response(pCR) was achieved in1.4%(1/70), completeresponse was2.9%(2/70), partial response was47.1%(33/70), no change was34.3%(24/70), the overall response was51.4%(36/70), and clinical benefit rate was85.7%(60/70),10patients had progress disease,38patients received gastric curativeresection. The tubular adenocarcinoma with an efficiency of62.5%, lowdifferentiation adenocarcinoma and signet ring cell carcinoma with an efficiency of32.14%, the difference has statistical significance(P<0.05). Multiple factors analysisshowed that histopathologic type and lymph node metastasis were the independentfactor which influenced neoadjuvant chemotherapy efficacy(P<0.05), but the age,gender, tumor size, histopathologic grade, etc were not correlated to neoadjuvantchemotherapy(P>0.05).[Conclusions]: Different histological types of gastric cancer have different responseand efficacy to neoadjuvant chemotherapy, tubular gland and papillary carcinomaneoadjuvant chemotherapy curative effect is better. Histological classification andlymph node metastasis is the effect of neoadjuvant chemotherapy for advanced gastriccancer of two independent factors, its importance is higher than the degree of differentiation, tumor size and other pathological factors,the efficacy of the lowdifferentiation adenocarcinoma and Signet ring cell carcinoma has worse than the welldifferentiated tubular adenocarcinoma, it may be of benefit in the selection oftreatment options in locally advanced gastric cancer.
