Objective: Analyze the clinical-pathological and molecular biological characteristicof gastrointestinal stromal tumor, and explore the prognosis-related factors that affectpatients with gastrointestinal stromal tumor undergoing R0resection.Methods:Collect the clinical-pathological parameters and molecular biologicalmaterials of79patients with gastrointestinal stromal tumor who underwent R0resection collected from Aug.2002to Dec.2008in The First Affiliated Hospital ofFujian Medical University were investigated retrospectively, Meanwhile, telephoneand retrospect outpatient reexamination to follow up the further result..Results: The follow-up rate was93.67%, and the median follow-up time was50(10-104)months. The overall disease-specific survival of79patients was96.47%at lyear,74.68%at3years and60.71%at5years.1、Univariate analysis indicated that the3-year and5-year survival rates of patientswith gastrointestinal stromal tumor of the stomach undergoing R0resection were bothwere higher than the patients that without gastrointestinal stromal tumors of thestomach;the3-year and5-year survival rates of patients that tumor size≤10cm, andmitotic count≤5/50HPF, and tumor capsule was completed, and CD34positive werehigher than the patients that whose size>10cm, mitotic count>5/50HPF, tumorcapsule was not completed, CD34negative; the3-year and5-year survival rates ofpatients that had taken imatinib mesylate after the radical operation were higher thanthe patients that had not taken imatinib mesylate after the R0resection; the3-year and5-year survival rates of patients that were very low-grade and low-grade in NIHgrading were both higher than the patients that medium-grade and height-grade; the3-year and5-year survival rates of patients with gastrointestinal stromal tumor of thestomach undergoing radical operation did not have obvious relationship with gender,age, CD117, S-100, SMA, Actin and Vimentin (P>0.05).2、Multivariate factors Cox analysis indicated that tumor site, tumor size, mitotic count, tumor capsule completed or not and imatinib mesylate taken or not after theradical operation were the important factors which affect prognosis of patients withgastrointestinal stromal tumor undergoing R0resection(P<0.05).Conclusion:Tumor site, tumor size, mitotic count, tumor capsule completed or notand imatinib mesylate taken or not after the radical operation were the importantindexs which can assess the prognosis of patients with gastrointestinal stromal tumorundergoing R0resection.NIH pathological risk grading contributed to the predictionof GIST malignant degree and prognosis.
