The Study Of HIV/STDs Related High Risk Behaviors And Social Network Among FSWs In Low Class Eestablishment From Three Different Prefecture Of Yunnan Province, China

Objective1. Explore FSWs in low class establishment HIV/STDs related high risk behaviors and related factors, so as to provide scientific evidence for behavior intervention targeting femal sex workers.2. To understand social network among FSWs in low class establishment from three different prefecture of Yunnan province and provide baseline datas for future study.Methods The survey methodology combine with quantitative research and qualitative research. The FSWs were selected by Snowball Sampling. The FSWs are divided into three group (K region representative of metropolis, the type of more foreign FSWs, the other two regions represent the type of more local FSWs, C is HIV highly endemic area P is HIV moderate popular area.) Sociodemographic characteristies, sexual behavior,guests, intervention services, knowledge of HIV and anamnesis about sexually transmitted diseases were collected by self-designed questionnaire after informed consensus. Totally,1082samples were successfully investigated.318samples were from K area,338samples were from C area and426samples were from P area. We use qualitative research method devise FSWs" year plan figure and Daily work figure. First,The tools is used for understand FSWs basic situation.Then the unstructured and non-participant observation is used for collecting information about the population, understand social network among FSWs in low class establishment from three different prefecture of Yunnan province.Results This survey questionnaires were received1082,of which were all valid questionnaires. There were93FSWs participate in group interviews and individual depth interviews, include20FSWs from K region,44FSWs from C region and44FSWs from P region. There is a big gap between three region of FSWs in sexual partner condom use. The data show that three regions more than80%FSWs using condom every time in the last months and it have significant differences (X2=530.187, P=0.000).Nearly60%FSWs "never use condom in the last months" when she have sex with intimate partner, and it have significant differences(X2=81.382, P=0.000).Different sex partner have different condom use impact factor.Conclusion Further targeted strengthen on FSWs in condom use with their intimate sex partners, to improve FSWs and intimate partner's HIV/AIDS prevention awareness and to reduce their risk sexual behavior.According to FSWs characteristics of social network, use of social network knowledge intervene sexual network high-risk groups (including clients, intimate partner etc) and then to achieve the purpose of preventing AIDS/STDS.Living conditions of FSWs in low class establishment generally poor. For social support, fight against discrimination, to assist FSWs. to reduce their negative psychological society.The result showed that whether use condom with clients in the last month (yes=Kno=2) related to FSWs? age(P=0.001). clients?age(P=0.O00, the cost of each sexual behavior(P=0.0018), whether to know correct of condoms can prevent HIV transmission(P=0.000), Whether to know only one sexual partner can reduce the opportunities of HIV infection(P=0.001), the frequency of condoms use with clients in the last month(P=0.000). Gynecological Disease Symptoms last years(P=0.000) and whether to follow AIDS education(P=0.008). Multivariate logistic regression analysis showed that clients'age, the frequency of condoms use with clients in the last month were independently associated with high risk behaviors among FSWs and clients, whether use condom with intimate sexual partner in the last month (yes=l,no=2) related to FSWs'age(P=0.009), marital status(P=0.039), who is you first sexual partner(P=0.008, whether to engage in commercial sex work at other palces(P=0.008), whether to accept HIV counseling and testing in recent year(P=0.038).Multivariate logistic regression analysis showed that FSWs· age, marital status, who is you first sexual partner whether to engage in commercial sex work at other palces were independently associated with high risk behaviors among FSWs and intimate sexual partner. FSWs in low class establishment social network generally divided into general social network and sexual network. Relative, frients, neighbors and peer make up FSWs general social network. The result showed that poverty is the reason FSWs engage sexual business and most of them had a part-time job. Sexual network include FSWs-, clients, intimate sexual partner(include boyfriend and husband) and so on. Low cost of each sexual behavior and large number of client lead to FSWs network get huge and have no border. Low literacy together with Poor health consciousness make people who are in the sexual network more likely to get AIDS.
