The Value Of Peripheral Artery Test In Coronary Artery Disease

[Objective] To analyse the correlation between the ankle brachial index (ABI), brachial ankle pulse wave velocity(Ba-PWV)and carotid intimal medial thickness (cIMT) and the severity of coronary atherosclerosis,so as to discuss the value of peripheral artery test in prediction of atherosclerosis and coronary heart disease.[Methods] All patients(n=197) were divided into CHD group(n=151) and coronary artery normal group(n=46) according to the results of coronary arteriography (CAG).CHD group was divided into single stenosis, double stenosis and severe group in term of the extent of coronary artery. All patients were given history collection, blood-lipid test.At the same time,cIMT was detected by high frequency color Doppler examination in GE Vivid7and whether have plaques, ABI and PWV was measured by Arteriosclerosis detector in Colin system.[Results] The comparison between two groups showed that in CHD group the ABI lower significantly(1.08±0.11,1.15±0.12),Ba-PWV(1858.79±418.61,1531.24±290.49) and cIMT(0.79±0.19,0.71±0.15) increased significantly. With the increasing of coronary lesions, ABI reduce, Ba-PWV and cIMT increased gradually. The proportion(44.4%) and incidence(23.5%) of ABI<0.9are higher in the severe group.The proportion(32.9%vs31.2%vs18.8%) and positive rate(90.3%vs96.4%vs94.1%) of Ba-PWV≥1400cm/s in CHD group are higher than the normal group(17.1%vs63.2%).The proportion (39.6%vs28.3%) of cIMT≥0.9mm significantly in double stenosis and severe group,the incidence(44.1%) is higher in severe group. The sensitivity and accuracy of PWV≥1400cm/s diagnosing coronary atherosclerosis were respectively93.4%and80.2%. ABI<0.9has higher specificity (93.5%). ABI joint cIMT and three methods above combined have the same specificity(97.8%).[Conclusion] ABI, PWV or cIMT is an effective method to predict the coronary atherosclerosis,has correlation with the severity of coronary artery stenosis closely.In CHD group ABI and cIMT, Ba-PWV and cIMT both have correlation. Ba-PWV≥ 1400cm/s has higher sensitivity and accuracy.ABI<0.9has higher specificity. ABI joint cIMT specificity increased, which is more advantageous to evaluate the coronary artery lesions early with better value of clinical application.
