Analysis Of Exercise-induced Hypotension For Diagnosis And Evaluation Of Coronary Heart Disease

Objective:To evaluate the diagnostic value of taking exercise-induced hypotension as a positive index in treadmill exercise test (TET) for coronary heart disease. Methods:We analyzed and compared the results of treadmill exercise test and coronary artery angiography (CAG) in201patients(men=86, women=115, Age=60.10±9.46yers) with suspected coronary heart disease, whom were hospitalized in the1st People's Hospital of Kunming, China from2009to2011. Results:(1)The diagnostic sensitivity, specificity and accuracy of TET for coronary heart disease were83.33%,48.27%and68.15%;(2) Exercise time and METs were significantly lower in the CAG-positive group and TET-positive group than that in the CAG-negative group and TET-negative group;(3)The diagnostic sensitivity, specificity and accuracy of Exercise-induced hypotension(EIH) for coronary heart disease were41.22%,66.66%and53.23%;(3) The positive rate of TET in coronary artery disease group was higher than that in normal coronary artery group (P<0.05);(4) The positive rate of TET in the group with coronary artery stenosis≥50%was higher than that in the group of coronary artery stenosis<50%(P<0.05);(5) The positive rate of CAG in the group with blood pressure decreasing early in the exercise was higher than that in the group of blood pressure not decreasing early (P<0.05);(6) The Logistic regression analysis for the risk factors of CHD such as TET, carotid artery plaque, age, sex, history of hypertension, history of diabetes, history of smoking, BMI, TC, TG, HDL-C, LDL-C showed Y=-1.319+2.563X13+1.634X12。Conclusions:(1) TET can be used to detect and eveluate coronary heart disease.;(2) Exercise-induced hypotension is an important indicator of treadmill exercise test for detecting and evaluating coronary heart disease.;(3) The early decreasing of blood pressure in exercise test should be of more significance for detecting and evaluating coronary heart disease;(4) TET and Echo-test for carotid artery plaque in carotid artery are important for CHD detection and evaluation..
