Objective: To invesgate the clinical application of ultrasound guidance in the treatment ofunresectable hepatic carcinoma with radiofrequency ablationMethod:90patients who were unfit for surgical therapy underwent RFA under ultrasoundguidence at Hepatobiliary Institute of Erpao General Hospital from March,2009toMarch,2012.They are arranged to three groups by the difference of guiding modality.Patientsaccepted LUS-guided RFA belong to group I, patients accepted PUS-guided RFA belong togroup II, patients accepted IOUS-guided RFA belong to group III.We study the clinical data ofthe patients and their condition in the operation,according to the datas,we can assess the theapplication value of ultrasound guidence in the treatment of unresectable hepatic carcinomaResults:1.RFA procedures were successfully completed in all patients.Comparing the volumeof blood loss and liquid supplied between Group I and Group II,there is no statisticaldifference(P>0.05).Comparing the total length of stay, postoperativ length of stay and thelength of surgery,there is statistical differenc P<0.05;2. Comparing the volume of blood lossand liquid supplied between Group I and Group III,there is statisticaldifference(P<0.05);Comparing the total length of stay èpostoperativ length of stay,the length ofsurgery between Group I and Group III,there is statistical differenc.3. Comparing the volumeof blood loss and liquid supplied between Group II and Group III,there is statisticaldifference(P<0.05).Comparing the total length of stay, postoperativ length of stay and thelength of surgery,there is statistical differenc P<0.054. The volume of blood loss and liquidsupplied, the length of surgery, total length of stay, postoperativ length of stay is the least inGroup II and the most in Group III.5.The total ablation rate, transaminase increase rate andAFP decrease rate between three groups were compared,there is no statisticaldifference(P>0.05). Conclusion:1. The application of ultrasound guidence can increase the accracy and safty ofthe treatment of unresectable hepatic carcinoma with radiofrequency ablation2.The handling ofLUS is convenient,LUS can increase the effect of the treatment of unresectable hepaticcarcinoma with radiofrequency ablation.3.In the process of PUS-guided RFA hepaticcarcinoma, the length of surgery is short,the loss of blood is few and the recover time ofpatients is short,but this modality is susceptible to gas of gestrointestinal and can not locate thelesion in the deeper location4.IOUS is not susceptible to gas of gestrointestinal,it can reveal thelesion without blind zone,but the length of surgery in the IOUS-guided surgery is long,the lossof blood and the volume of liquid supplied is more and the recover time of patients is long.
