Objectives:To observe the effect and side effect of the new proton pump inhibitor(PPI)-Esomeprazole(Nexium) on complex peptic ulcer, compared with Omeprazole(Losec).Methods:61 complex peptic ulcer pateints were randomly divided into experimental and control group(EAC and OAC), the former (n=31)was treated by Esomeprazole 40 mg once daily, and the latter (n=30)was treated by Omeprazole 40 mg once daily, the course of treatment are 6 weeks, none of other antacids is used. The HP-positive patients synchronously received Clarithromycin 500mg plus Amoxicillin 500mg for 14 days, Clarithromycin and Amoxicillin were given twice daily. To evaluate the release of the gastrointestinal symptoms such as upper abdominal pain,acid reflux,heart burn after treatment by symptom grades, ulcer healing by gastroendoscopy, HP eradication by 14C-UBT test. To observe drug side effects of Esomeprazole and Omeprazole. Results: HP eradication rate in EAC group and OAC group were 91.3% and 96.4%, respectively; there was no significant diffirence between two groups bychi-square test(P>0.05). Ulcer healing rate in EAC group and OAC group were 87.1% and 83.3%, respectively; no significant diffirence between two groups by chi-square test(P>0.05). The release rate of upper abdominal pain in EAC group and OAC group is 88%,42.3% respectively; The release rate of acid reflux in EAC group and OAC group is 85.2%,40% respectively; The release rate of heart burn in EAC group and OAC group is 80.8%,38.5% respectively; Esomeprazole was prior to Omeprazole on the effect of relieving upper abdominal symptoms(upper abdominal pain,acid reflux,heart burn), there was significant diffirence between EAC group and OAC group(P<0.05). Side effects of Esomeprazole and Omeprazole was slight, with no significant diffirence between two groups. Conclusion: For complex peptic ulcer, Esomeprazole was similar to Omeprazole on the ulcer healing rate and HP eradication, with slight side effects. The former was prior to the latter on relieving upper ganstrointestinal symptoms (upper abdominal pain,acid reflux,heart burn). Esomeprazole was an ideal remedy for complex peptic ulcer.
