Objective:To explore the impact of different CYP2C19 genotypes on healing rate of peptic ulcer by omeprazole in children.Methods:From January 2017 to August 2019,we recruited 44 children with peptic ulcer confirmed by digestive endoscopy who were hospitalized in Shenzhen Children’s Hospital.The clinical data of and CYP2C19 genotypes were collected.25 cases were extensive metabolizers(EM group),10 cases immediate metabolizers(IM group)and 9 cases poor metabolizers(PM group).Children in IM group and PM group were treated with standard dosage of omeprazole(0.6-0.8mg/kg/d).Within EM group,12 cases were treated with standard dosage of omeprazole 0.6-0.8mg/kg(EM standard-dosage group)and 13 cases with high dosage of omeprazole(0.8-1.3mg/kg/d)(EM high-dosage group).The healing rate of peptic ulcer was compared between groups.Results:1.The ulcer healing rate was 80.0% in IM group and 88.9% in PM group,respectively.It was significantly higher than 25.0% in EM standard-dosage group(P<0.05).2.The ulcer healing rate in the high-dosage group(76.9%)was significantly higher than the standard-dosage group within EM group(25.0%)(P<0.05).Conclusions:1.CYP2C19 genotypes has an impact on healing rate of peptic ulcer by omeprazolein in children.2.High-dosage omeprazole may improve outcome of peptic ulcer in extensive metabolizers with homozygous genotypes.
