Objective To evaluate the clinical values of insulin-like growth factor 1((IGF-1)combinded with carcinoembryonic antigen(CEA),cytokeratin fragment antiogen 21-1 (CYFRA21-1),neuron-specific enolase(NSE)in diagnosis and prediction of response to therapy in human lung cancer.Methods The serum levels of IGF-1 were detected by radioimmunoassay in 91 lung cancer patients,30 health adults and 15 patients with benign pulmonary disease. The serum levels of CEA,CYFRA21-1 and NSE were measured by electrochemiluminescence immunoassays in same persons. Diagnostic efficacy of these tumor markers were evaluated by ROC curves.Results The four kinds of serum tumor markers levels in lung cancer group were obviously higher compared with benign disease group and health group(IGF-1:x~2=26.95,P<0.001,CEA:x~2=49.11,P<0.001;CYFRA21-1:x~2=40.63,P<0.001;NSE:x~2=14.76;P<0.001). Only IGF-1 and CYFRA21-1 showed significant difference between patients before treatment and after treament(IGF-1: x~2=5.99,P=0.014;CYFRA21-1:x~2=4.99,P=0.025). No significantt differences were found for IGF-1between 3 histological types in lung cancer(x~2=0.03,P >0. 05);The sensitivity of CYFRA21-1 was higher than othor markers for squamous-cell carcinoma (x~2=9.09,P<0. 05),and the same was observed in NSE for the diagnosis of small cell lung cancer (x~2=1.81,P<0.05). The sensitivity of IGF-1,CEA,CYFRA21-1 and NSE was75.5%(34/45),53.3%(24/45),66.7%(30/45)and 42.2%(19/45) when the specificity was 95%(43/45),respectively . Their area under the ROC curve (AUC) was 0.88,0.836,0.891and 0.697,respectively. The sensitivity of IGF-1 was highest among them in diagnosing lung cancer. The combination of IGF-1 with CEA ,CYFRA21-1,NSE is the more sensitive than other markers used alone in diagnosing lung cancer.Conclusion IGF-1,CEA,CYFRA21-1 and NSE are valuable parameters in diagnosing lung cancer and IGF-1 can be used to judge response to treatment. Combination of IGF-1 with CEA,CYFRA21-1,NSE enhances significantly the sensitivity and efficiency in diagnosing lung cancer. The combination of IGF-1 with CYFRA21-1 or the combination of IGF-1,CYFRA21-1and CEA is useful in diagnosing lung cancer.2.The early dignostic value of IGF-1 combinded with CEA,CA199,CA125 in gastrointestinal malignant tumorsObjective To evaluate the early dignostic values of insulin-like growth factor 1((IGF-1)combinded with carcinoembryonic antigen(CEA),carbohydrate antigen 199(CA199),carbohydrate antigen 125 (CA125 )in gastrointestinal malignant tumors.Methods The serum levels of IGF-1 were detected by radioimmunoassay in 46 patients with gastrointestinal malignant tumors,30 health adults and 19 patients with benign gastrointestinal diseases. The serum levels of CEA,CA199 andCA125 were measured by electrochemiluminescence immunoassays in same persons. Diagnostic efficacy of these tumor markers were evaluated by ROC.Results The four kinds of serum tumor markers levels in gastrointestinal malignant tumors group were obviously higher compared with benign disease group and health group((IGF-1:x~2=25.448,P<0.001;CEA:x~2=29.798,P<0.001;CA199: x~2 =8.071,P=0.004;CA125:x~2=17.779,P<0.001). Only IGF-1 and CA125 showed significant difference between patients before treatment and after treament(IGF-1:x~2=5.643,P =0.018;CA1251:x~2=5.210,P=0.022). The sensitivity of IGF-1,CEA,CA199 and CA125 was 64.0%(16/25),50.2%(13/25),48.0%(12/25)and 40%(10/25)when the specificity was 93.9%(46/49),respectively . Their area under the ROC curve (AUC) was 0.838,0.822,0.767and 0.679,respectively. The sensitivity of IGF-1 was highest among them in diagnosing gastrointestinal malignant tumors. The combination of IGF-1 with CEA ,CA199,CA125 is the more sensitive than other markers used alone in diagnosing gastrointestinal malignant tumors. Conclusion IGF-1 are valuable parameters in diagnosing gastrointestinal malignant tumors and IGF-1 can be used to judge response to treatment. The combination of IGF-1 with CEA ,the combination of IGF-1,CEA with CA199 or the combination of IGF-1,CEA,CA199 with CA125 are useful in diagnosing gastrointestinal malignant tumors.3.The comporative study of diagnositic value of IGF-1 combinded with serum tumor markers CEA and CYFRA21-1 and 18F-FDG PET/CT on lung cancerObjective To evaluate the diagnositic value of IGF-1 combinded with serum tumor markers CEA and CYFRA21-1 and positron emission tomography/ computed tomography with fluorine-18-labeled fluoredeoxyglucose (18F-FDG PET/CT) on lung cancer.Methods 18F-FDG PET/CT scan was performed in 18 patients with lung cancer and 1 patients with benign pulmonary disease.The serum levels of IGF-1,CEA and CYFRA21-1 were detected in the same persons. The acurate diagnosis rate and false positive rate of the combination of IGF-1,CEA with CYFRA21-1 and PET/ CT were analyzed.Results The acurate diagnosis rate and false positive rate of the combination of IGF-1 with serum tumor markers CEA and CYFRA21-1 were 78.9%( 15 /19) and 16.7%( 3 /18), respectively; and those of 18F-FDG PET/CT scan were 89.5% (17 /19)and11.1%(2/18), respectively.Conclusion The combination of IGF-1 with serum tumor markers CEA and CYFRA21-1 is an effective cancer screening tool of health examination and follow-up method on lung cancer. 18F-FDG PET/CT is advantaged in the diagnosis , Staging and pinpoint of lung cancer.
