Objective: To analyze the metastasis of lymph node in gastric antrum cancer. The aim is to investigate the need for the dissection of superior mesenteric vein (14v) lymph node in radical gastrectomy for gastric antrum cancer in China.Methods: It is a retrospective study of 183 cases of gastric antrum cancer in our hospital from January 2008 to December 2010. Analyze metastasis of superior mesenteric vein (14v) lymph node and discuss the value of dissecting superior mesenteric vein (14v) lymph node in the standard radical gastrectomy for gastric antrum cancer (D2).Results: In 183 patients, 12 patients had a superior mesenteric vein (14v) lymph node metastasis with the rate of 6.6%. The metastasis of superior mesenteric vein (14v) lymph node was not significantly related to patient's age, gender and the general form of the tumor, but had a relation with the invasion depth, the size and the differentiation degree of tumor. All 12 patients with superior mesenteric vein (14v) lymph node metastasis had No.6 lymph node metastasis. The patients with No.6 lymph node metastasis had superior mesenteric vein (14v) lymph node metastasis with an average of 14.5%.Conclusion: The metastasis rate of superior mesenteric vein (14v) lymph node in gastric antrum cancer is 6.6%. The patients with a low degree of tumor differentiation are more likely to metastasize to superior mesenteric vein (14v) lymph node. The tumor of the patients who has a superior mesenteric vein (14v) lymph node metastasis often invades gastric serosal. There is a close relationship between the metastasis of superior mesenteric vein (14v) lymph node and that of No.6. No.6 lymph node can be a clinically effective and useful indicator for the dissection of superior mesenteric vein (14v) lymph node.
