Objectives: To determine the effects of low-dose aspirin use on the endometrial thickness and the outcome of pregnancy in the application of different periods in nonselected i n in-vitro fertilization-embryo transfer(IVF-ET) / intracytoplasmic sperm injection (ICSI) cycles.Method: This study was a prospective ,randomized controlled trial. One hundred and sixty infertile patients who were received treatment in Dalian Maternity Hospital Reproductive Center from March 2009 to July 2009 in this study. All patients underwent IVF / ICSI treatment with conventional long protocol after oral contraceptives pill.Excluded the patients with aspirin contraindications disease before treatment.Patients with cardiovascular disorders, and those taking medications that could affect the circulation were also excluded. Approval for the study was obtained from the Clinical Research Ethics Committee of the institution. All patients were voluntary and signed informed consent before the treatment cycle.A totle of 160 Infertile women with long protocol cycles were randomized into four groups . oral aspirin(75mg/d) from the day of oral contraceptives pill in group A(n=40), oral aspirin(75mg/d) from the first day of gonadotrophin-releasing hormone agonist in group B(n=40), oral aspirin(75mg/d) from the first day of gonadotrophin stimulation in group C(n=40),or received no treatment in group D(n=40).All treatment was continued until the day before embryo transfer.Result: Our results showed that ,there were no significant differences recorded in the duration of stimulation, gonadotropin consumption, peak estradiol levels and follicles that were>14 mm in diameter on the day of human chorionic gonadotropin administration(P>0.05).The endometrial thickness in the four groups respectively were group A:9.72±1.27mm,group B:9.62±1.40mm,group C:9.23±1.99mm,group D :8.91±1.71mm , groupA and group B were significantly higher than group D (respectively; P<0.01). And there were no statistically significant differences between the group A and group B(P>0.05). The number of oocytes retrieved in the four groups respectively were group A: 8.53±3.16,group B:8.58±2.55,group C:8.30±2.69,group D :8.53±2.71,there were no significant difference between the four groups (respectively;P> 0.05). The number of embryos retrieved,number of embryos transferred, number of frozen embryos were no significant difference between the four groups (respectively;P> 0.05). The number of fertilization rate in the four groups respectively were group A: 75.60±19.96,group B:74.53±18.86,group C:75.24±13.26,group D : 75.33±17.72,there were no significant difference between the four groups (respectively;P> 0.05). The number of cleavage rate in the four groups respectively were group A:97.78±14.79,group B:99.44±14.77,group C:95.88±10.20,group D :98.46±10.25,there were no significant difference between the four groups (respectively;P> 0.05). There was a increase in the overall implantation and clinical pregnancy rates,and there was a decrease in the overall abortion rate with the increase in administration time in four groups,but also no significant differences(P>0.05).Conclusion: Adjuvant treatment with low-dose aspirin does not affect the controlled ovarian hyperstimulation outcomes and embryo quality in assisted reproductive technolo.Can significantly improves the endometrial thickness ,maybe increase its receptivity,so that the implantation and clinical pregnancy rates has increased,and the abortion rate has decreased.Adjuvant treatment with low-dose aspirin from the day of injected gonadotrophin-releasing hormone agonist and treatment with oral contraceptives pill,also can be significantly increased endometrial thickness, and between the two groups had no significant difference.Adjuvant treatment from the day of gonadotrophin-releasing hormone agonist both to increased the endometrial thickness,but also reduced the medication time Comparison with Adjuvant treatment from the day of oral contraceptives pill,is a relatively appropriate starting delivery time.
