Objective:The aim of this study was to explore the relationships between Visfation,IL-1β,Leptin in follicular fluid (FF) of women treated with in vitro ferti1ization and embryo transfer( IVF-ET) and IVF-ET results through the correlation recearches between them and oocytes retrieved,fertility rate,rate of hign quality embryos, also to evaluate their effection and clinical application value in IVF-ET, so as to provid theoretical evidence and open a new way for better diagnosis and treatment of infertility.Method:1.We collected 54 cases of women treated with IVF-ET as the first group of research subjects whose causes of the disease are pelvic tubal factor or male factor .They were divided into pregnant group and non-pregnant group after determining the clinical pregnancy. And the second group includes 18 PCOS women treated with IVF-ET.2.All of the patients recived the same controlled ovarian stimulationl (standard length of the program) for IVF-ET. Serum was collected from empty stomach IVF-ET patients in the morning on the day when they underwent oocyte retrieval.Follicular fluid was collected from patients undergoing oocyte retrieval of IVF-ET.All the specimens stored in -80℃in the refrigerator after centrifugation to detect related factors.3.Visfatin level in serum and the levels of visfatin,IL-1β,leptin in FF were detected using ELISA.The detection of progesterone (P) and estradiol (E2) in FF use chemilumines- cence.4.Analysis the relationships between visfatin,IL-1β,leptin in FF and the number of oocytes, fertilization rate and the rate of good embryos.5.Analysis expression differences of visfatin in serum, visfatin,IL-1β,leptin in FF between PCOS patients with infertility and non-PCOS with infertility. Results:1. The levels of visfatin and IL-1βin FF were found significantly higher in pregnant group than non-pregnant group(22.17±3.61ng/ml vs 18.87±2.51ng/ml, 45.75±10.14pg/ml vs 38.13±9.12 pg/ml;P<0.05).The levels of leptin in FF in pregnant group was significantly lower than non-pregnant group(24.19±5.71ng/ml vs 29.26±7.14ng/m1,P<0.05).2. In the bivariate Pearson correlation analysis:FF visfatin levels was positively correlated with BMI (r=0.586, P<0.05) , the numbers of retrivaled oocyte (r=0.686, P<0.05) and fertilization rate (r=0.622, P<0.05); FF IL-1βconcentration was positively correlated with the numbers of retrivaled oocyte (r=0.686, P<0.05),fertilization rate (r=0.533, P<0.05)and high-quality embryo rate (r=0.473, P<0.05);also so as visfatin(r=0.486, P<0.05);FF leptin concentration was positively correlated with BMI (r=0.505, P<0.05), but negativly correlated with E2 (r=-0.474, P<0.05), high-quality embryo rate (r=-0.422, P<0.05).3. There is no significantly correlation between PCOS and non-PCOS patients in the levels of visfatin in the serum and in FF (PCOS patients: r=0.237, P>0.05; non-PCOS patients:r=0.122; P>0.05).4.The content of visfatin in serum and FF were significantly higher (25.10±2.54ng/ml vs 20.06±4.74ng/ml; 20.54±3.45 ng/ml vs 16.91±2.55ng/ml, P<0.05 ) in PCOS group than non-PCOS group, and so as leptin in FF (28.96±7.50ng/mL vs 20.07±5.15ng/ml, P<0.05),.The content of IL-1βin FF of PCOS patients has no significant difference from control groups (P>0.05).Conclusion:1.Visfatin exists in follicular fluid.There are no correlation between visfatin in serum and in FF.2. FF visfatin levels are positively correlated with the numbers of oocytes and fertilization rates, and the FF visfatin concentration in pregnant group is significantly higher than in non-pregnant group; speculated that the content of visfatinin FF reflects the ovarian response, and as a predictor of IVF pregnancy outcome indicators.3.IL-1βlevels in FF are positively correlated with the numbers of oocytes, fertilization rates and high quality embryo rates , inferred that IL-1βcan be used as a predictor of embryo quality and IVF pregnancy outcome.4. IL-1βand visfatin levels in FF were positively correlated, suggesting that visfatin may be effect the pregnancy outcomes of IVF-ET by upregulating expression of IL-1β.5.The presence of visfatin in serum, visfatin and leptin in FF are increased in PCOS patients, showing that visfatin, leptin associated with the pathogenesis of PCOS.
