Objective: to compare the clinical results of OCM minimally invasive and modified Hardinge approach for total hip replacement.Methods: during January 2005 and February 2008,the patients suffered with total hip replacement were divided into two groups in the same period, OCM approach was performed in 30cases and modified Hardinge approach was performed in 35cases , during an average of (50.9±10.1) months follow-up. All operations were done by same group of surgeons.Clinical evaluation involved the harris hip score and Barthel index were in the 1 st week ,the 1 st month ,the 3 rd month , the 6 th month and every year . Radiographs were examined for prosthesis position,abduction angle.All the complication in both groups was recorded and statistically analyzed for all patients.Result: The incision length, blood loss, perioperative transfusion and 100-mm VAS score at the first 24 hours of OCM group were significantly low. The Harris hip score and Barthel index were significant high in OCM group at 3 months follow-up and became equal to modified Hardinge group at the 50.9 th month after operation.Conclusions: Both OCM minimally invasive and modified Hardinge approach be successfully used for total hip replacement , But the recovery of the OCM approach of minimally invasive total hip replacement within three months after operation was the the better.
