Objective: To evaluate the application value of laparoscopic surgery in early endometrial cancer.Methods: According to FIGO 1988, retrospective analysis 91 cases which are diagnosised as early endometrial cancerⅠby diagnostic curettage, during Jan. 2007 and Dec. 2009 in Dalian Maternity Hospital. Patients were divided as two groups on their desire and economics. There are 37 cases in treatment group (laparoscopic surgery) and 54 cases in control group (laparotomy). Folowed up for 12-48 months. Comparing with the blood loss, the amount of lymph node dissection, the time of surgery, the postoperative exhaust time, the time of temperature recovery, the rate of complications during and postoperative, the average hospital stays, the survival rate in 2 yeays, disease-free survival and postoperative recurrence rate.Results: Blood loss in laparoscopic surgeries was (320.11±256.20)ml, and (556.75±324.62)ml in laparotomy, (P<0.05); the amount of lymph node dissection was (24.53±12.33) in laparoscopic surgeries, and (30.41±11.17) in laparotomy, (P<0.05); There was significant difference between the two groups, (P<0.05); The time of surgery in laparoscopic surgeries and laparotomy were (200.47±53.87)min. and(211.88±48.90) min. (P>0.05); the postoperative exhaust time in laparoscopic surgeries was (1.41±0.86)d, and(2.97±1.47)d in laparotomy,(P<0.05); The time of temperature recovery were (4.94±3.16)d and (5.47±2.97)d, (P>0.05); There are 2 cases recurrenced in laparoscopic surgeries (5.4%) and 13 cases recurrenced in laparotomy(24.1%), there was significant difference between the two groups, (P<0.05); The average hospital stays are(12.22±6.19)d and(18.72±6.84)d. The difference has statistical significance (P=0.002); There are 2 relapse in laparoscopic surgeries and 1 death.the postoperative recurrence rate is 5.9% and the disease-free surviva is 94.1%, the survival rate is 97.1%. There are 3 relapse in laparotomy and 2 death, the postoperative recurrence rate is 5.8% and the disease-free surviva is 94.2%, the survival rate is 96.2%. There is no significant differences, (P>0.05).Conclusion: There is less blood loss compared with traditional operations and less rate of complications. There are some traits such as quick intestinal tract function recovery, less time in hospital. Without more relapse rate and death rate. The short-term efficacy is satisfying. It's a preferable modus operandi in management early endometrial cancer. We can use laparoscopic surgeries as a conventional treatment technic.
