objective: Observation of functional endoscopic sinus surgery with sevoflurane and propofol effectiveness and safety of controlled hypotension.Methods:40 cases of ASAⅡ-III grade elective functional endoscopic sinus surgery patients, randomly divided into two groups(n=20). Group A is controlled hypotension group, Group B is non-controlled blood pressure group. Group A at the beginning of the operation to maintain mean arterial pressure (MAP) 70 mmHg for hypotension, to the end of surgery; Group A and B with anesthetic drugs, but B group the line controlled hypotension. According to Fromme surgical field quality score, recorded by the antihypertensive drugs did not know the same surgeon in all patients before anesthesia in surgery (T1),immediately before surgery(T2),surgery began 30 min(T3),the end of surgery(T4),10 min after the end of surgery(T5) evaluation of the surgical field quality,intraoperative continuous monitoring MAP,HR,ECG and SpO2 ,record operation time, intraoperative ECG changes after surgery blood gas analysis taken 30min.Results:Group A of T3 and T2 when MAP decreased significantly when compared(P<0.01)and returned at T2. Group A of T3, MAP also decreased compared with the Group B was significantly(P<0.01); The two groups at each time point was no significant difference between HR(P<0.01);Group A of blood loss and operative time was significantly less than the Group B(P<0.01);no arrhythmia in both groups and myocardial ischemia, postoperative and preoperative electrocardiogram showed no significant change; two groups after 30min of arterial blood gas analysis were within normal limits.Conclusion: Functional endoscopic sinus surgery with sevoflurane and propofol induced hypotension can reduce bleeding and improve surgical conditions of the operative field to minimize the operative time, no obvious side effects and complications. Sevoflurane reduced the mean arterial pressure 65mmHg - 70 mmHg,can reduce the nasal mucosa microcirculation,compared to other commonly used vasodilator effect of antihypertensive drugs better, for endoscopic sinus surgery is safe, effective and feasible, more conducive to combined heart, liver, kidney and other diseases in patients with endoscopic treatment.
