Objective: The routine examination and corneal topography were taken to observe the incisions of different type, the change of visual acuity,the astigmatism and the corneal topography preoperatively and postoperatively.Mothods:The 40 examples of patients (46eyes) with phacoemulsification and foldable posterior chamber introcular lens(IOL) implantation were collected in our hospital, and were divided into two groups randomly. Group A of 20 cases 20 eyes were treated with phacoemulsification through sclera tunnel incisions and group B of 20 cases 26 eyes were treated with hyaline corneal incision. All patients underwent phacoemulsification and foldable IOL implantation, at superior (12:00), and the dimension is 3.0mm, and the class of nuclear isⅡ-Ⅲof class in two groups. The routine examination and corneal topography were taken to observe the change of vision,astigmatism and corneal topography pre-operation,1 day,1 month and 3 months after operation.Results: The visual acuities between group A and the group B preoperatively , there was no significant difference, ( P>0.05 ). 1 day after operation , the visual acuities in group A were better than those of the group B, there was significant difference between 2 groups, ** ( P<0.05 ). 1 day,1 month and 3 months after operation , the visual acuities in two groups were better than pre-operation , there was significant difference between 2 groups , * ( P<0.05 ). 1 month and 3 months after operation , the visual acuities between group A and the group B, there was no statistically significant difference( P>0.05 ). The astigmatism between group A and the group B preoperatively , there was no significant difference preperatively(P>0.05) . 1 day,1 month and 3 months after operation , the astigmatism in group A were less than those of the group B**( P<0.05 ). 3 month after operation, the astigmatism changes in two groups were less than pre-operation , there was significant difference between 2 groups* ( P<0.05 ). 1 day after operation,it becomes steeper that incision at the meridian between two groups. The color of corneal topography becomes warm color relative to pre-operation. Along with time slip, the astigmatism decrease in two groups. The majority of incisions (12:00) corresponding with surrounding of cornea becomes relatively smooth. The cornea curvature becomes small, and the color of corneal topography becomes cool color at the meridian, and there was no significant change at the color of corneal topography on the other parts after operation . 1 month,3months after operation, There was no obvious change between two groups of corneal topography .Conclutions:1. The sclera tunnel incisions has the advance which is smaller change of corneal topography,smaller astigmatism. 2. The sclera tunnel incisions has the advance which is quicker visual recovery and more safety as well as hyaline corneal incision. 3. The earlier period after operation, the visual acuities in the sclera tunnel incisions were better than the hyaline corneal incisions. The long period after operation, there was no significant difference between two groups. 4.The incision which at the steepest meridian can decrease corneal curvature, and reduce corneal astigmatism to some extent.
