Objective: Explore the therapeutic effect of the treatment of femoral shaft fracture in intramedullary interlocking nail and compression plate.Methods:69 cases of femoral shaft fracture admitted from September 2007 to October 2009 were reviewed retrospectively. According to the way randomly,69 cases divided into group compression plates and group locking intramedullary nail. group compression plates was 33 cases, group locking intramedullary nail was 36 cases. Operation time, hospital stay, fracture union time, blood loss and knee dysfunction were compared between this two groups. Using statistical analysis from the SPSS14.0 software analyzes the data. Analysis and comparison of Postoperative body recovery time,Body weight and functional exercise.Results: All patients were followed up for 2-26 months,average 13 months. compared blood loss and fracture union time, blood loss of group locking intramedullary nail was less than group compression plates. fracture union time of group locking intramedullary nai was better than group compression plates〔P<0.05〕. group locking intramedullary nai was 36 cases,1 cases appeared infection. Postoperative 1 months, knee joint activities of 5 cases is limited, hip activities of 11 cases limited. Postoperative 3 months, knee joint activities of 2 cases is limited. Postoperative 6 months, knee buckling mobility of 1 case is 90 degrees. All patients were using an unreamed deice treatment, and all osseous healing, the average of fracture union time is 113.1 d. Postoperative patients all did not appear fracture delayed union or not heal. efficacy evaluation:34 cases is excellent,1 cases is good and1 cases is middle.group compression plates is 33 cases, 2 cases appeared infection, 1 cases appeared bone nonunion, 2 cases appeared plates broken because of weight premature. Postoperative 1 months, knee joint activities of 8 cases is limited, hip activities of 3 cases limited. Postoperative 3 months, knee joint activities of 5 cases is limited. Postoperative 6 months, knee buckling mobility of 4 case is 90 degrees. efficacy evaluation:30 cases is excellent,2 cases is good and 1 cases is middle. postoperative infection of femoral fractures is about 2.5%,higher than intramedullary nails technology. hospitalization time have difference between two groups of patients (p﹤0.05). knee function recovery time have difference between two groups of patients (p﹤0.05).Conclusion:1.According to the principle of AO and BO, femoral shaft fracture should use open reduction plate fixation,it good for anatomical reposition of femur, early functional exercise and fracture union,two methods can all reach the above requirement.2. Clinical studies showed that intramedullary nail fixed compared with compression plates therapy has its unique advantages: less fracture malunion happened,function Improved,early recovery work, less hospital stay,and less fracture union time. Compared with open reduction,it has fewer complications and reduced infection rates.3. fixation of intramedullary interlocking nail is the preferred surgical method for various femoral fractures which is between femoral small roto and 9cm above the knee clearance.
