Objective:To explore the diagnostic value of detection of BNP combined with survivin in malignant pleural effusions.Methods:Pleural effusion samples are obtained from 83 patients who presented to the respiratory (26 with tuberculous pleural effusion,30 with malignant pleural effusion,14 with cardiac origin pleural effusion,and 13 with parapneumonic effusion). The real time quantitative RT-PCR was used to detect the expression of Survivin mRNA.The pleural effusion levels of BNP were determined by sandwich Enzyme linked immunosorbent assays (ELISA).Results:(1)The pleural concentrations of BNP in cardiac origin (2119.1±858.92 pg/ml) group was significantly higher than those in malignant (557.27±172.6pg/ml). tuberculous (339.08±98.89) groups and parapneumonic effusion(306.33±107.51), there was statistical significance among all four groups(p<0.05). The pleural concentrations of BNP in malignant group was higher than those in tuberculosis and parapneumonic effusion, there was statistical significance in pleural concentrations of BNP (p<0.05).The expression of Survivin mRNA in malignant group was significantly higher than those control groups, there was statistical significance (p<0.05). Pleural BNP level was positive correlation with survivin mRNA. (2)By analysis of ROC curves, the cut-off calues for BNP was defined. The sensitivity and specificity of BNP for malignant effusion were 70% and 89.5%.Conclusion:Detecting of BNP in pleural effusion may be useful in the diagnosis of tuberculous and malignant pleural effusion. The expression of Survivin mRNA in malignant group is significantly higher than other groups. The combined detection can be used for diagnostic purpose.
