Objective:Pleural effusion is a common disease of respiratory medicine,Is emergeoften because the liquid in the pleural cavity to generate too much, too fast, or absorb tooslow. Malignant pleural effusion is a common complication of lung cancer and othermalignant tumor, treatment is difficult and the prognosis is poor. Therefore, timelydiagnosis and treatment can be able to relieve patients’ symptoms and prolong survivaltime. This study by detecting survivin (Survivin) and carcinoembryonic antigen(carcinoembryonicantigen, CEA) Situation expression in benign pleural effusion (mainlytuberculosis) and malignant pleural effusion expression. To explore the both in benign andmalignant pleural effusion diagnostic valueMethod: Collected July2012-December201380cases in our hospital patients withpleural effusion and clinical data included in this study, Benign pleural effusion group of36cases,44cases of malignant pleural effusion. By the method of enzyme-linkedimmunosorbent (enzyme-linked immunosorbent assay, ELISA) respectivelydetermination Survivin and CEA concentration in the two pleural effusion groups,Results:1. Malignant group (44cases) Survivin, CEA concentrationrespectively (86.12±14.92ng/ml,54.42±11.33ng/ml), benign group (36cases)respectively(40.47±10.06ng/ml,3.04±1.39ng/ml). malignant group Survivin,CEAconcentration were significantly higher than in benign group corresponding indicators,using t test between two groups, the difference is statistically significant(p<0.01);2.Survivin, CEA in the AUC value were0.905,0.883, obtained by the ROC curve. and thebest diagnostic value were52.63ng/ml,5.89ng/ml, and the sensitivity of diagnosis was77.27%,68.18%; Specific degree86.11%,91.67%respectively, both joint detectionsensitivity of94.45%,77.78%.3. The comparison of two groups of patients diagnosis using x2test, there was no statistically significant difference between the two groups(P>0.05)Conclusion:1、Survivin and CEA in the diagnosis of benign and malignant pleuraleffusion has a certain value, Survivin and CEA is not significant difference betweendiagnostic rate. The joint detection can improve the sensitivity of the diagnosis, but thespecificity is reduced.
