Objective: In this paper,diabetic patients are divided into 3 groups by detecting urinary albumin excretion rate (UAER),meanwhile, by investigating the serum concentration of ghrelin, resistin and adiponctin at different stages of diabetic nephropathy,comparativing analysis of each group serum ghrelin, resistin and adiponctin levels,and researching the relationship between ghrelin, resistin and adiponctin and DN, study the change and the role of ghrelin, resistin and adiponctin in the the occurrence and development of diabetic nephropathy,in order for an in-depth to explore the pathogenesis of diabetic nephropathy,in order to provide some new ideas for the diagnosis and treatment of diabetic nephropathy.Methods:Select hospital patients with type 2 diabetes 90 cases and 30 cases of healthy control group in this study. First, measure the UAER level of all selected objects,and according to UAER(μg/min),diabetes will be divided into three groups:30 cases with simple diabetes mellitus group(SDM group,UAER<20),30 cases with early diabetic nephropathy group(EDN group, 20≤UAER≤200),30 cases with clinical diabetic nephropathy group(CDN group,UAER>200).Using enzyme-linked immunosorbent assay the test object is measured in serum ghrelin, resistin and adiponctin levels,simultaneous,glucose,insulin,lipids,renal levels are detected. Using analysis of variance and q test to compare the group differences in levels of ghrelin, resistin and adiponctin, Pearson correlation analysis the relationship between ghrelin, resistin, adiponctin and various clinical indicators,multiple linear stepwise regression analysis the influencing factors of diabetic nephropathy.Results:The serum level of ghrelin, resistin and adiponctin increased gradually with the development of disease in patients with type 2 diabetes (P<0.05 or P<0.01), the ghrelin and adiponctin levels in DM group decreased than that in the control group, the differences were statistically significant(P<0.05).Pearson correlation analysis showed that Serum ghrelin in the diabetes was positive correlation with TG,UAER and urea nitrogen,and was negative correlation with body mass index and C-P(P < 0. 05). Serum resistin in the diabetes was positive correlation with HbA1C,TG,LDLc,BUN and UAER,and was negative correlation with FINS and HDL (P < 0. 05). Serum adiponectin in the diabetes was positive correlation with UAER and HDL-c,and was negative correlation with BMI,TG,FPG (P < 0. 05).With ghrelin as the dependent variable, other parameters as variables in multiple linear stepwise regression analysis,the results of the variables into the equation are BMI,UAER and BUN,the HbA1C,TG,BUN,UAER seemed to be the best variable in predieting resistin ,the UAER and BMI seemed to be the best variable in predieting adiponectin.Conclusions:1.With the progression of diabetic nephropathy,the content of ghrelin,resistin and adiponectin in serum gradually increased. Increased blood serum ghrelin,resistin and adiponectin not only plays a role in the development of diabetic nephropathy,but possibly also has great meaning during diagnosis,prevention and treatment. 2. Compare with normal control group,reduced blood serum ghrelin and adiponectin are associated with in type 2 diabetes mellitus. 3. Ghrelin,Resistin and Adiponectin may be involved in the development of diabetic nephropathy and plays an important role in its occurrence and development process.
