OBJECTIVE: To investigate the serum adiponectin and urinary albumin excretion rate(UAER), blood-fat in type 2diabetic patients with different stages of nephropathy, and the relationship between of these factors.METHODS: 60 type 2 diabetic patients with nephropathy from the department of endocrinology in our hospital and 30 normal healthy people. According to UAER , these patients were divided into three groups: normal albuminura, microalbuminura, macroalbuminura group. The level of serum adiponectin, HbAlc, blood-fat, UAER of these object were measured and compared.RESULTS: The level of serum adiponectin was higher in normal healthy people than type 2 diabetic patients with nephropathy [(9.69 + 2.23mg/l)vs(2.17 + 1.95mg/l)] and was highter in microalbuminura group than normal albuminura group[(3.81 +1.7mg/l)vs(0.74 + 0.47mg/l) ], and the serum adiponectin concentration in the macroalbuminura group was significantly highter than normal-and microalbuminura groups(p<0.001). Univariable analysis showed serum adiponectin to be positively associated with UAER and HDL (r=0.91 vs r=0.31) and to be negatively associated with BMI and HbAlc (r=0.71 vs r=0.32). Multiple regression analysis showed UAER to be the highest influence.CONCLUSION: There are glucose and lipid metablic disorder and insulin resistance, which is one of the major factors in the lower serum adiponectin, However, with the increase of UAER, the level of serum adiponectin is actually increased.
