Clinical Study Of Low Dose Thalidomide Combined With Only One-Week's Dexamethasone's VAD Regimen On Multiple Myeloma

ObjectiveMultiple myeloma(MM)is one of malignant plasmacyte disease of blood system diseases. The overall response rate is 50%-60% with conventional VAD regimen in treatment of MM,and the complete remission is 3%-13%.But there is a large dosage of dexamethasone in VAD regimen,and it is more likely to infection.Thalidomide is one of angiogenesis inhibitor. In this paper, we compare the efficacy and toxicity of low dose thalidomide combined with only one-week's dexamethasone's VAD regimen in treatment on patients with MM,and compare with standard regimen.Methods and meterials42 patients with MM were diagnosed in our hospital. the only one-week s dexamethasone's VAD regimen is Vincristione 0.4mg·d-1, Adramycin 10mg·d-1, Dexamethasone 40mg·d-1.for 4 days for 1 cycle. At the same time, thalidomide is given 150mg-200mg·d-1.ResultsAmong the 42 evaluable patients,32 cases of newly diagnosed,10 cases of progressive diease. The overall response rate is(ORR) 80.95%, in concluding 6 cases(14.28%) achieved complete remission (CR),20 cases (47.62%) achieved partial remission(PR),8 cases (19.05%) showed improving and 8 cases (19.05%) had no effects. There is small adverse reactions,including drowsiness in 20 cases(47.62%),Dizziness in 5cases(11.9%),constipation in 30 cases (71.43%) numbness of the extremities in 24 cases(57.14%),lower extremities in lease (2.4%), Leukopenia in 10cases(28.31%).Conclusionslow dose thalidomide combined with only one-week's dexamethasone's VAD regimen in treatment of MM has more merits such as less side effects,good tolerance,convenient administration and significant curative effects,it would be valuable to utilize in clinical widespread.
