Objective Orthotopic splenectomy is a new tendency recently years which was improved based on traditional splenectomy. This research is to compare and analyse hemorrhage and complications of orthotopic splenectomy and traditional splenectomy by retrospective study.Method Collecting and looking up the illness records of hepatic surgery center of Wuhan Tongji hospital from January 2006 to August 2010 which was diagnosed HC and PH and picking the records merely had splenectomy. The total records are 98 and according to different surgery methods is divided into two groups. The orthotopic splenectomy group have 46 records ,and the traditional splenectomy group have 52 records. Obtaining the clinical data and doing statistical analysis.Results The blood loss of the orthotopic splenectomy group and the traditional splenectomy group are 281.63±109.30ml and 422.45±153.00ml(P<0.05). The orthotopic splenectomy group have no pancreatic fistula and the traditional splenectomy group have 5(9.61%) P<0.05. The orthotopic splenectomy group have 11(31.42%) need transfusion and the traditional splenectomy group have 27(51.92%), P<0.05.Conclusions Compared with traditional splenectomy, orthotopic splenectomy can reduce the blood loss and transfusion in the operation and reduce the incidence of pancreatic fistula. Orthotopic splenectomy is superior to traditional splenectomy in merely splenectomy patients of HC and PH.
