Objective To investigate genotype distribution and frequency of allele for adiponectin(APN) gene at 45 and 276 locus in Han nationality population in Ningxia. To investigate the relevance of gene polymorphism and metabolic syndrome (MS) at above two locus; To investigate the correlation of MS patients'adiponectin gene polymorphism with coronary heart disease; To investigate what factors does the genetic polymorphism at the two locuseffects and then impacts the occurrence and development of MS and CHD; To explore the relationship of APN gene, plasma APN levels, metabolic syndrome and coronary heart disease.Metohds 102 healthy controls (control group), 107 patients with metabolic syndrome ( MS group) , 93 patients with both metabolic syndrome and CHD (M + C group ) according to the selection criteria were included in this study. To inspect all groups'polymorphism at the second explicit branch area (+45 locus) and the second introns area (+276 locus) by adopting experiment - research method and Polymerase chain reaction - restriction fragment length polymorphism (PCR-RFLP) technology. The blood levels of APN were quantified using sandwich ELISA and determined FINS by radioimmunoassay. To figure up body mass index (BMI) according to the formula weight (kg) / height(m) 2, calculate insulin resistance index HOMA-IR = (FBG X fasting insulin)/22.5 by steady-state model evaluation method. And then evaluate the differences and relationship of the index from three groups through using statistical methods such as variance analysis,χ2 inspection, t-test and Logistic regression analysis.Result1.Comparison of three groups' clinical and biochemical indicators; There is no big difference on age and gender (P > 0.05) in three groups. Compared with the normal control group, MS group and M+C group's BMI SBP, BMI, DBP, FPG, TG, TC, LDL - C, WHR goes up, HDL-C goes down obviously (P < 0.05). Compared wi th MS group, M+C group's fasting plasma glucose (FPG), glycerine three cool (TG) raises further, high-density lipoprotein (HDL) reduce further (P < 0.05).2.The comparison of the three groups'plasma APN, homa-ir level. Compared with the normal control group, MS group and M + C group's APN level reduces, plasma HOMA-IR increases with significant difference (P < 0.05). Compared with MS group, M + C group's HOMA-IR increases further (P < 0.05).3.Distribution of three groups'of genotype and allele frequencyAt SNPS + 45 locus: there is great difference for three groups'genotype and allele distribution frequency,χ2 = 14.951, P = 0.005;χ2 = 7.083, P = 0.029 (P < 0.05). Compared with the normal control group, there isn't great difference for MS group's genotype and allele distribution frequency,χ2 = 3.229, P = 0.199;χ2 = 0.356, P = 0.551(P > 0.05); Compared with the other two groups, the genotype frequency of M+C group on SNP+45TT locus goes up obviously, M+C group on SNP+45 TG+GG locus goes down,χ2=4.507,P=0.034;χ2 =14.276,P=0.001;χ2=4.069,P=0.044;χ2=6.784,P=0.011(P<0.05)。At SNPS + 276 locus: there is no significant difference on genotype and allele distribution frequency for all three groups,χ2 = 2.338, P = 0.624;χ2 = 1.463, P = 0.481 (P > 0.05).4.The comparison of APN, HOMA-IR level of different genotypes adiponectin. The genotype APN level of SNPS + 45 TG + GG goes up compared with TT type; The genotype APN level of SNPS + 276 TG + TT is much lower compared with GG type, (P < 0.05). There is no great difference on HOMA-IR level (P > 0.05) for people with different genotype.5.The relationship between polymorphism of adiponectin gene and MS Taking MS as dependent variable and gene polymorphism at SNPS + 45, SNPS + 276 locus , age, gender, SBP, DBP, FPG, TG, HOMA-IR as independent variables, carry through multiple factors gradually Logistic. Regression analysis shows that FPG, BMI, SBP, TG, SNPS + gene polymorphism at 276 locus are risk factors to effect MS. That who carries 276TT genotypes is easy to affect MS; there is no relationship between SNPS + 45 gene polymorphism and MS.6 The analysis on the relationship between the polymorphism of adiponectin gene and components of MSTaking MS groups as dependent variable, alleles, blood pressure, FPG, BMI, TG at SNP+45, SNP+276 as independent variables, proceeding multiple linear regression (MLR) analysis, gene polymorphism enters regression equation taking HOMA-IR as dependent variable. It indicates allele at SNP+276 locus increases the risk of elevation of blood pressure, resistance of isulin. It's the dangerous factor to affect MS; Polymorphism at SNPS + 45 locus doesn't enter into regression equation.7. The correlation between adiponectin gene polymorphism of MS patient and CHD. On MS and M + C group, taking whether merge CHD as dependent variable(yes = 1, no = 0), gene polymorphism at APN SNPS + 45, SNPS + 276, age, gender, SBP and DBP, FPG, TG, homa-ir as independent variables, proceeding multiple factors gradually Logistic, regression analysis shows that SBP, FPG, high LDL - C are dangerous factors to affect CHD. Carry SNP SNPS + 45G alleles, high hdl-c hematic disease are protection factor to against CHD.Conclusion1.The level of APN reduced for MS patients Han nationality in Ningxia.2.SNP +45 TT genotype frequency of CHD patients was significantly increased, SNP +45 TG + GG genotype and G allele frequencies were significantly reduced.3.The level of adiponectin for those carriers of adiponectin genotype SNPS + 45 TG + GG goes up, for those carriers of SNPS + 276 TG + TT goes down.4.People who carries the SNP + 276T alleles would run more risk on affecting MS, risk for those who carries SNPS + 45 G decreases.5.People who carries the SNPS + 45G alleles would reduce the occurrence on affecting CHD by increasing the level of APN.6. People who carries the SNP + 276T alleles would run more risk on affecting MS by APN level reducing, insulin resistance aggravating and blood pressure elevating.
