Objective. To investigate the clinical characteristics of viral encephalitis (VB), and to assess the related factors which affect the prognosis during hospitalization . Methods A retrospective clinical study was performed about 168 cases with viral encephalitis between 2005 and 2009 . Factors influencing the prognosis were evaluated by using univariate and multivariate Logistic analyses.Results. A total of 168 cases were analyzed: 94 were males ,74 were females. Most of the patients were adolescent and middle-aged person. The mean age was (29.05±16.17) years, 56.5% patients lived in the countryside, the rest worked in the city. As many as 90.5% patients were acute or subacute duration; 67 patients were respiratory infection before the ill .presenting symptoms and signs included : behave change(64.3%) Fever (76.8 %), headache (52.4%), seizure (51.2%), meningeal irritation (57.1%) and focal neurological deficits (22.6%) ,52% samples of CSF were normal ,, most of the rest were slight increased with white blood cells and protein, glucose and chloride were also in the normal range; Abnormalities were found in 52.2% of patients who had MRI data, which were higher than CT from the sensitivity viewpoint . The prognosis results were divided into two groups .109 patients had good outcome, and 79 patients had poor outcome. Univariate and multivariate Logistic analysis showed that older ages, focal neurological deficits ,the decline of consciousness ,without antiviral therapy were risk factors related with poor prognosis.Conclusions Early diagnosis of vital encephalitis (VB) is difficult. In the clinical ,diagnosis is based on a combination of clinical features, laboratory tests and neuroradiological imaging .Adequate antiviral therapy should be timely given with suspicion of vital encephalitis , patients would probably have higher case fatality when they appeared with older ages, focal neurological deficits ,the decline of consciousness ,without antiviral therapy.
