Object:1.To study the relationship of immune-related and metabolic-related with Kazakh's esophageal cancer in Xinjiang;2.To evaluate the role and significance of the HLA-DRB1*0901,TAP2379,TAP1637 and MTHFRC677T as a screening index with EC in Kazakh's.Method:1.To screen the cases as the gold standard of gastroscope and histopathology check. All the 125 cases were collected from the six hospitals in north of Xinjiang during the periods from November 2006 to October 2010. Screen the 250 controls from the date who were outpatients without history of malignant tumor or non-esophageal lesions during the same period as cases.2. CTLA-4 genetic polymorphisms were identified from the blood samples by PCR-RFLP; subjoin the date of HLA-DRB1*0901,TAP2379,TAP1637,MTHFRC677T and CYP2E1 by PCR or PCR-RFLP.3. All the calculations and statistics were performed with the computer program, SPSS Version 13.0. Statistical analysis adopt conditional logistic regression to calculate the values ofχ2, odds ratios and 95% confidence intervals for possible risk factors; to detect the results of HLA-DRB1*0901,TAP2379,TAP 1637 and MTHFRC677T alone or combined in Kazakh's esophageal cancer.Results:1. Univariate conditional Logistic regression analysis showed:HLA-DRB1*0901 allele,TAP2379,TAP1637, MTHFRC677T genetic polymorphisms could increase the chance of suffering from EC in Kazakh of Xinjiang. The OR were1.919(95%CI:1.273-2.892); 1.882(95%CI:1.323-2.677); 1.717(95%CI:1.199-2.442); 1.853(95%CI:1.240-2.769); 1.425(95%CI:1.004-2.024).The frequencies of CTLA-4+49A/G genotype in case group is not different from the controls(χ2=0.275,P=0.600).2. Multivariate conditional Logistic regression analysis showed:except the CYP2E1 genetic polymorphism,HLA-DRB 1*0901 allele,TAP2379,TAP1637, MTHFRC677T genetic polymorphisms could increase the chance of suffering from EC in Kazakh of Xinjiang. The OR were 1.620(95%CI:1.065-2.464); 1.715(95%CI:1.201-2.449); 1.473(95%CI:1.020-2.126); 1.526(95%CI:1.011-2.303).3. The sensitivity of HLA-DRB1*0901,TAP2379,AP1637 and MTHFRC677T in single detection: 24.0%,44.8%,58.4%,74.4%;the specificity:90.8%,77.2%,61.6%,45.6%.4. Pairwise respectively detection of HLA-DRB1*0901,TAP2379,TAP1637 and MTHFRC677T,the higher sensitivity is in united detection of MTHFRC677T and TAP1637, their sensitivity and specificity of paralleling test are 88.0%,32.0%5. In united detection of immune-related gene HLA-DRB1*0901,TAP2379 and TAP 1637, the sensitivity and specificity of paralleling test are 83.2%,45.2%;in united detection of HLA-DRB1*0901,TAP2379,TAP1637 and MTHFRC677T, the sensitivity and specificity of paralleling test are 91.2%,23.6%.Conclusion:1. HLA-DRB1*0901 allele,TAP2379,TAP 1637 and MTHFRC677T genetic polymorphisms are related to esophageal cancer in Kazakh of Xinjiang.CTLA-4+49A/G and CYP2E1 genetic polymorphisms is not related to esophageal cancer in Kazakh of Xinjiang.2. MTHFRC677T could be used as the screening index with EC in Kazakh's of Xinjiang.3. The paralleling test detection of MTHFRC677T and TAP1637 could be used as the screening index with EC in Kazakh's of Xinjiang,they could increase the sensitivity 13.6%.
