| Objective:Investigate patients'dental anxiety from the impacted wisdom tooth extraction at general hospitals and analyze relevant factors. Evaluate the clinical efficacy of music intervention and cognitive insight intervention in removal of impacted wisdom tooth with dental anxiety in patients.Methods:1. General factor questionnaire, Modified Dental Anxiety Scale and State Anxiety Inventory were used to assess the removal of impacted wisdom teeth patients and analysis of relevant factors.2. The patients were randomly divided into music intervention group, cognitive insight intervention group and non-intervention group,40 cases were studied in each group. Routine treatment of non-intervention group, not to implement any intervention; In addition to conventional treatment, the intervention group Implemented the music intervention or cognitive insight intervention. Modified dental anxiety scale, state anxiety inventory and hamilton anxiety scale were used to assess dental anxiety index before and after tooth extraction, and assess postoperative pain of three groups patientsResults:1. Removal of impacted wisdom teeth in 300 patients, the incidence of dental anxiety was 56%.2. May cause dental anxiety on the 10 factors were analyzed by univariate analysis, there are 7 factors associated with dental anxiety. Further multivariate analysis:5 factors into the regression equation, They are gender, only child, childhood dental experiences, fear of pain and the onset of wisdom teeth pericoronitis form。The relationships were more between gender, fear of pain and dental anxiety.3.1n this study, removal of impacted wisdom teeth were selected 120 patients,40 cases o f music intervention group,40 cases of cognitive insight intervention group and 40 cases of control group. After statistical testing, there is not statistically significant of three groups in gender, age, occupation, education and other general information (P>0.05). Three groups of patients were assessed the degree of pain and dental anxiety before surgery, there is no statistically significant difference (P>0.05). Three groups of patients were comparable.4. After music intervention, the average scores of MDAS and S-AI were statistically significant differences before and after surgery in patients (P<0.01). HAMA score less points, HAMA reduction rate was 31.52%. The average score of MDAS, S-AI and HAMA were no significant differences before and after surgery in non-intervention group (P>0.05).5. After cognitive insight intervention, the average scores of MDAS and S-AI were statistically significant differences before and after surgery in patients (P<0.01). HAMA score less points, HAMA reduction rate was 31.57%. The average score of MDAS, S-AI and HAMA were no significant differences before and after surgery in non-intervention group (P>0.05).6. After music intervention, the average scores of pain was statistically significant differences before and after surgery in patients (P<0.01). The average score of pain was no significant differences before and after surgery in non-intervention group (P>0.05).7. After cognitive insight intervention, the average scores of pain was statistically significant differences before and after surgery in patients (P<0.01). The average score of pain was no significant differences before and after surgery in non-intervention group (P>0.05).Conclusions:1. There was high prevalence of dental anxiety in patients with removal of impacted wisdom teeth, clinicians should be given attention.2. Dental anxiety is sensitive to women, only child, childhood dental experiences, fear of pain and Wisdom tooth pericoronitis patients with recurrent than men, than non-child, without childhood dental experiences, not afraid of pain and first occurred wisdom teeth pericoronitis in patients.3. Music intervention may be reduce the dental anxiety and relieve pain in patients of impacted wisdom tooth extraction and help to improve patient mood.4. Cognitive insight intervention may be reduce the dental anxiety and relieve pain in patients of impacted wisdom tooth extraction and help to improve patient mood. |
