BackgroundThere has been a rapid increase in the population newly infected with HIV (Human Immunodeficiency Virus) among MSM (men who have sex with men) in recent years in China, The battle against HIV among MSM population faces several challenges including risk behaviors despite sufficient knowledge of HIV, low condom use and unprotected sexual intercourses. "Sero-sorting" is a method used in foreign countries to reduce the harm of HIV spread among MSM in recent years. The definition of "sero-sorting" is;" people choose sex partner with the same infection status to have unprotected sex."ObjectiveTo figure out whether "sero-sorting" has emerged spontaneously among MSM in southwest China. To analyze the influential factors of "sero-sorting", and to discuss the feasibility and methods of promoting "sero-sorting".MethodChengdu, Chongqing, Guiyang and Kunming of southweast China were selected as study sites, Questionnaire survey and Individual in-depth interviews were made with MSM at the sites.ResultAmong the 436 MSM surveyed,56.0% would inform his own infection status when looking for homosexual partner,74.3% would ask about the partner's infection status, and 77.1% would purposely select homosexual partner with same infection status. Sero-sorting was considered feasiable by 43.1% of the participants. Among the 41 HIV positive MSM,73.2% would purposely do "sero-sorting".41.5% HIV positive MSM would continue a relationship if the partner is HIV positive whereas refuse sexual act if the partner is HIV negative.34.1%HIV positive MSM have regular homosexual partner who is also infected.Factors that are associated with "sero-sorting" are:occupation, time from first date to sex with homosexual partner, and whether inform and ask a partner about one's infection status when looking for homosexual partner. When controlling other factors, whether inform a partner about one's own infection status when looking for homosexual partner is the most important factor.ConlusionThe findings show that "sero-sorting" has spontaneously appeared among MSM in southwest China, in the form of HIV- positive MSM looking for homosexual partner in network of HIV- positive MSM established by local MSM grass-roots organizations. We can propaganda "sero-sorting" to HIV-positive MSM through internet. In addition, local CDCs need to provide VCT, HIV care and new education materials to the target population.
