ClassⅡdivisionⅠmalocclusion is very common clinically, seriously affect oral function and facial appearance. The key point is how to open bite quickly and effectively and improve the overjet relationships. This is also the therapeutic nodus. Some research showed that Tip-Edge appliance could open bite promptly, simply, efficiently and comfortably with differential force theory for teenage patients. But for adult patients, many remain unclear. How is the therapeutic effect? What changes happened in dentition, occlusion and maxillofacial region? What is the mechanism? Could soft tissue profile be improved quickly? Could patients'confidence be reinforced? Answering these questions could provide alternatives for orthodontists in treating adult patients with classⅡdivisionⅠmalocclusion. Our research observed the changes in dentition, occlusion and maxillofacial region after the first stage treatment by Tip-Edge appliance in adult patients of ClassⅡdivisionⅠmalocclusion, investigated the mechanism and provided reference for clinical treatment of adult patients of ClassⅡdivisionⅠmalocclusion. In this study, we selected 20 adult patients with ClassⅡdivisionⅠmalocclusion, treated with Tip-Edge straight wire appliance and observed the clinical effect regularity. In the first stage, we aligned upper and lower teeth, corrected deep overbite and overjet, and then compared cephalometric radiograph before and after the first stage. Our research could provide information for understanding the mechanism of treating adult patients with ClassⅡdivisionⅠmalocclusion. We further investigated the mechanism of dental and skeletal changes in orthodontic process.We divided this experiment into two parts.1 Clinical Study on the first stage treatment of ClassⅡdivision 1 malocclusion adult patients by Tip- Edge techniqueObjective:To investigate the first stage treatment of ClassⅡdivision 1 malocclusion adult patients by Tip- Edge technique and explore the treatment mechanics. Methods:20 cases of Class II division 1 adult patients were treated by Tip- Edge technique in clinic. Cephalometric analysis was made before and after first stage treatment. Paired test was employed for the statistical analysis with SPSS16 software. Results:The average period of the first stage treatment is 4.6 months. After the first-stage treatment, molar and canine relations of these patients were in the normal range. Cephalometric analysis shows that UI-NA and U1-SN reduced 13.68(°P<0.01)and 12.25(°P<0.01), UIC-PP reduced 1.32mm(P<0.05), U1-NA reduced 4.16mm(P<0.01), LI-MP reduced 2.82mm(P<0.05),LIC-MP reduced 2.08mm(P<0.05),overbite were reduced 3.94mm, overjet were reduced 2.03mm(P<0.01). Other indices were no significant changes. Conclusion:Tip- Edge technique can be used effectively for deep bite correction by anteriors intrusion and correct the ClassⅡdivision 1 malocclusion adult patients comfortably and rapidly. 2 Analysis of soft tissue profile changes before and after the first stage treatment by Tip- Edge techniqueObjective:To investigate the first stage treatment of ClassⅡdivision 1 malocclusion adult patients by Tip- Edge technique and analyzed soft tissue profile changes before and after treatment. Methods:20 cases of Class II division 1 adult patients were treated by Tip- Edge technique in clinic and then analyzed soft tissue profile changes after treatment. Paired test was employed for the statistical analysis with SPSS16 software. Results:Patients'soft tissue profile improved significantly in the first stage. Soft tissue Cephalometric related nose angle measurements showed an average increase of 8.72°(P<0.01), Z increased by an average angle of 6.52°(P<0.01); ULP,LLP reduced 2.54mm(P<0.01)and 2.32mm(P<0.01), upper lip increased by an average angle of 0.99mm(P<0.05), and other indicators in soft tissue were no significant changes. the relationships between nose, lips and chin were more harmony, and the profile of the soft tissues were improved. Conclusion:The adult patients of Angle's class II division 1 have the clinical demond for impovement of facial tissue profile. Opening the bite early can reduce the overbite and overjet, while camouflage the convex profile rapidly and effectively, thus the patients'confident for later treatment were heightened.
