Objective Lubar disc degeneration was considered clinically as the pathological basis of lumbar disc herniation,lumbar canal stenosis and chronic low back pain .The mechanism of Disc degeneration was more complex, The present study of disc degeneration was manifested the changes in cells and components of extracellular matrix,and the latter was the direct reason of the loss of the mechanical properties of the disc.In short,the essence of disc degeneration was the change of extracellular matrix and apoptosis caused by adherent function decrease of cells and matrix.This study discussed the function of apoptosis in degenerated lumbar intervertebral disc by measuring the expression of Caspase-3 and XIAP ptotein and analyzed the relationship between the prominent lumbar intervertebral disc tissue and the ratio of sagittal decreased diameter.Method 58 cases of patients with orthopaedic diagnosis of lumbar intervertebral disc herniation were selected as experimental group,24 cases without orthopaedic diagnosis of lumbar intervertebral disc herniation were selected as control group.all the cases were collected from The Affiliated Hospital of Qinghai University between October 2009 and October 2010.Caspase-3 and XIAP ptotein were measured by immunohistochemistry SP and TUNEL, then analyzed the relationship between them. Result The positive rate of Caspase - 3 in the nucleus pulposus cells was higher than that in the control group (P < 0.01); The degree of nucleus pulposus cells apoptosis in experimental group was negatively related to the expression of XIAP ptotein (P < 0.05), and was significantly correlated to the expression of Caspase - 3 (P < 0.01).The content of caspase-3 and XIAP ptotein in nucleus pulposus and prominent objects were not correlated to the ratio of sagittal intradiscal diameter (p > 0.05) .Conclusion Caspase-3 participated and promoted the development of lumbar intervertebral disc degeneration, which played an important role in the pathogenesis of lumbar.The expression of XIAP ptotein in degenerated nucleus suggested that body regulated the apoptosis to protect the cells from the nucleus pulposus apoptosis by raising inhibitory factors. Inspection and analysis confirmed the significant correlation between the two groups.Further study of the regulatory mechanism of apoptosis and disc degeneration can help to elucidate the pathogenesis of intervertebral disc and provide new targets for clinical interention.
