Estimate Of The Cardiovascular Disease Risk Of The Newly Diagnosed Adult Latent Autoimmune Diabetes

Objective:The study aimed to investigate the level of high sensitivity C-reactive protein (hs-CRP) and carotid intima medial thickness (CIMT) in the newly diagnosed adult latent autoimmune diabetes (LADA) and newly diagnosed type 2 diabetes mellitus (T2DM),and then to estimate the cardiovascular disease risk of LADA, also to compare the differences between newly diagnosed LADA and newly diagnosed T2DM in cardiovascular disease risk.Methods:132 cases were in endocrinology of Jilin Province People's Hospital from August 2009 to August 2010,and they were all newly diagnosed diabetes. After determination of diabetes autoantibodies,132 cases newly diagnosed diabetes were divided into LADA group (n=60) and T2DM group (n=72). At the same time, selected 46 cases from physical examination center in normal controls group (NC group), whoes age and body mass index matched with LADA group.All of the cases were measuring blood pressure, height, weight, and testing for liver function, blood lipid, fasting blood glucose(FBG),2-hour postprandial blood glucose(2hPG), glycated hemoglobin, hs- CRP and so on. All of the patients were measuring carotid intima medial thickness (CIMT) to decide with or without atherosclerosis (AS), then selected LADA without AS group(n=40), T2DM without AS group(n=52). NC group excluded patients with AS.Results:1.The concentration of hs-CRP and the levels of CIMT in the LADA group was significantly higher than NC group[(2.46±1.00)VS(1.23±0.86),(0.93±0.36)VS(0.57±0.17),P< 0.01-0.05], and in LADA without AS group also higher than NC group [(1.90±0.68)VS(1.23±0.86),(0.74±0.14)VS(0.57±0.17),P<0.05]. The risk stratification of cardiovascular disease using the levels of hs-CRP, the percentage of moderate risk group and high risk group in the LADA group was markedly elevated than NC group(X2=26.658, P< 0.001;X2=13.127,P< 0.001).2.Through compared the levels of hs-CRP and CIMT, there were no significant difference between LADA group(2.46±1.00;0.93±0.36) and T2DM group(2.29±1.04;0.91±0.40), also between LADA without AS group (1.90±0.68;0.74±0.14)and T2DM without AS group(1.82±0.80;0.72±0.15). There were no statistical significance between LADA group and T2DM group in the incidence of AS and carotid atherosclerosis plaque(X2=0.478,P=0.489; X2=0.010,P=0.920), the proportion of moderate risk group and high risk group of cardiovascular disease between two group were also no significant difference(X2=0.776,P=0.378>0.05;X2=0.017,P=0.896>0.05).3.In LADA group, LDL-C, FPG, HbAlc,2hPG, hs-CRP showed a significant linear correlation with CIMT(P=0.001-0.044). Further multiple regression analysis, the multiple linear regression analysis equation was CIMT =-0.961+0.095*2hPG+0.163*hs-CRP。Conclusion:1. The elevated hs-CRP levels in newly diagnosed LADA indicated that it was yet in a chronic inflammatory state, and its risk of cardiovascular disease was increased.2. The elevated CIMT levels in newly diagnosed LADA indicated that its risk of cardiovascular disease was increased.3. The hs-CRP and CIMT levels in newly diagnosed LADA and newly diagnosed T2DM was no significant difference, suggesting that their risk of cardiovascular disease were no significant difference.
