Purpose:To investigate the prevalence and clinical features of pulmonary hypertension (PH) in end-stage renal disease (ESRD) patients undergoing hemodialysis (HD), and compare with peritoneal dialysis (PD) patients, for early diagnosis and intervention.Patients and Methods:clear, complete information of 58 ESRD patients in the Department of Nephrology in Provincial Hospital Affiliated to Shandong University between August 2006 and January 2011 were retrospectively analyzed,30 patients receiving maintenance hemodialysis (MHD) treatment,28 patients received peritoneal dialysis (PD) treatment. Diagnosis of PH depended on echocardiography.Results:There are 10 males and 20 females in 30 HD patients, mean age is (60.9±10.6) years, mean dialysis age is (12.29±11.73) months,43.33% having PH; 13 males,15 females in 28 PD patients, mean age is (58.1±7.1) years, mean dialysis duration is (15.53±16.08) months,14.29% having PH, which was significantly lower than that in HD patients. The proportion of HD patients who use arteriovenous fistula (AVF) was significantly higher in PH patients than those without PH, and interval dialysis weight gain was significantly higher than those without PH. There were no significant difference of age, gender, dialysis duration, blood pressure, primary disease, Hb, Hct, BUN, Scr, ALB, Ca, P, calcium and phosphorus product and PTH between PH group and non—PH group (all P<0.05). Multivariate analysis revealed only interval dialysis weight gain was independent factor (P<0.05)Conclusions:PH is a common complication in HD patients, which is rare in PD patients. So when ESRD patients who have increased risk for the development of PH need renal replacement therapy, starting with PD may be a reasonable option.
