Background and ObjectivesMultiple sclerosis (MS) is a demyelinating disease characterized as multiple lesions and the remitting relapsing courses, recent studies have shown that autoimmunity may be the main pathogenic mechanism.Although immunopathogenic mechanisms of MS initiation and progression were incompletely understood, members of several protease families, termed the MS degradome, likely contribute, klkl and klk6 are a subfamily of serine proteases that have defined or proposed roles in a range of central nervous system (CNS) and non-CNS pathologies. These novel findings suggest that klkl and klk6 may serve as serological markers of MS and contribute directly to the demyelination and axonal injury. Given the abundance of kallikreins in serum and the potential utility of serum sampling to monitor MS disease activity, our goal in this study was to determine the relationship between the serum klkl and klk6 levels and the relapse and the disease severity of patients with multiple sclerosis.Material and MethodsFifty MS patients with initial attack (age:35.2±5.4, range 15 to 50 years) and fifty MS patients with recurrent attack (age:30.7±7.1, range 16 to 52 years) were enrolled in the study in the neurology department of the frist affiliated hospital of ZhengZhou university from 2007 to 2010. All MS patients were diagnosed according to the McDonald's creteria, all of them were laboratory-supported defined or clinical defined MS,and determined the Expanded Disability Status Scale (EDSS)of initial attack MS patients (3.8±1.4) and recurrent attack MS patients (4.4±1.7). Fifty other neurological diseases patients (age:36.1±6.3, range 19 to 58 years) and fifty healthy volunteers (age:34.1±7.3, range 16 to 56 years) constituted control groups. All subjects were well informed of all experimental purposes and signed a consent form, and all of them were drown peripheral venous blood 3 ML and the blood samples were preserved at the temperature -70℃. The sandwich-type ELISA was used to determine serum klkl and klk6 levels. The expression of the Statistical analysis results were mean±standard deviation(x±s),and the data analyzed with spss 13.0 statistical software. One-way ANOVA test was used to compare different groups. Correlation analysis utilize Pearson product-moment or Spearman rank correlation, and the significant level was set at P<0.05.Results1. The klkl serum concentration of the MS patients with initial attack and patients with recurrent attack were significantly higher than that of other neurological diseases controls (P<0.05) and normal controls (P<0.05),and the serum k1k1 concentration of the recurrent group was also elevated significantly compared to the initial group; the serum concentration of klkl shown no significantly difference between other neurological diseases controls and normal controls.2. The klk6 serum concentration of the MS patients with initial attack and patients with recurrent attack were significantly higher than that of other neurological diseases controls (P<0.05) and normal controls (P<0.05),and the serum klk6 concentration of the recurrent group was also elevated significantly compared to the initial groups; the serum concentration of klkl shown no significantly difference between other neurological diseases controls and normal controls.3. There were no significant correlation between the EDSS scores of the MS patients and the klkl serum concentration (P>0.01). The EDSS scores of the MS patients were no significant correlation to the klk6 serum concentration (P>0.01) 4. Serum klkl level and disease recurrence were positively correlated(r=0.667 P <0.01), Serum klk6 level and disease recurrence were also positively correlated (r=0.500 P<0.01).Conclusions1. The klkl serum concentration of the MS patients were significantly higher than that of controls, the data indicates that klkl may play a role during the course of MS.2. The serum klkl concentration of the recurrent group was elevated significantly compared to the initial groups, and serum klkl level and disease recurrence were positively correlated, the data indicates that klkl take part in the relapse of MS.3. The klk6 serum concentration of the MS patients were significantly higher than that of controls, the data indicates that klk6 may play a role during the course of MS.4. The serum klk6 concentration of the recurrent group was elevated significantly compared to the initial groups, and serum klk6 level and disease recurrence were positively correlated, the data indicates that klk6 take part in the relapse of MS.
