| Objective:With the development of economy, there is a increasing number of obese patients. Obesity is becoming a problem of public health. It is easy for obese paients to have hypertension, diabetes mellitus, and cardiovascular disease. In some researches conducted in recent years, obesity can induce kidney disease and albunminuria. The fundamental research showed that there is a relation between the low of the serum adiponectin and albunminuria. Therefore the objective of this study is to explore the correlation between serum adiponectin and urine albumin in patients with simple obesity in Tianjin by measuring the level of microalbunminuria and the serum ADPN, and offer the evidence for the clinical diagosis and treatment of obesity-related glomerulopathy.Methods:The simple obesity group(OB) were 60 cases. The control group(Con) were 20 cases. measure their body height, weight, waist circumference(WC), hip circumference, systolic blood pressure, diastolic blood pressure,and calculate body mass index(BMI) and waist to hip ratio (WHR) and mean arterial pressure(MAP) At the same time, Serum of these people were collected. Adiponectin(ADPN) was measured by enzyme-linked immunosorbent assay. Leptin(Lep) was measured by radioimmunoassay. Glucose(Glu) was measured by glucose oxidase method. Creatinine(CRE), uric acid(UA), and urea nitrogen(BUN) were measured by enzymic method. Total cholesterol(TC), triglyceride(TG), high density lipoproteins-c(HDL-c), low density lipoproteins-c(LDL-c) was measured by enzymic method. Urine microalbumin(ALB) and urine creatinine(UCR) were measured by radio immunoassay. Then calculate the ratio of urine microalbumin and urine creatinine (ACR). The patients in OB were allocated three groups by BMI: the first group:<35, the second group:35-40, the third group:>40. To compare the level of serum ADPN and urine ALB in groups.The patients in OB were allocated three groups by serum ADPN:A组>4ng/ml, B组3-4 ng/ml, C组<3ng/ml. To compare the level of urine ALB in groups. The patients in OB were allocated two groups by sex.Then to compare the level of serum ADPN and urine ALB in groups.Results:1 The level of glucose and the level of blood pressure were similar, and there was no significant(MAP:85.85±8.79 vs 90.58±7.04,Glu:4.88±0.90 vs 5.29±1.18, P>0.05). The level of serum ADPN in OB group was lower notably than that in the healthy adults group, and the difference was statistically significant(3.52±1.53 vs 4.95±0.25, P<0.01). The level of urine ALB in OB group was higher notably than that in the control group,and the difference was statistically significant(23.06±22.52 vs 7.26±6.48,P<0.01).2 The comparison of the positive rate of albuminuria in obese patients:It was 20% in the whole obese group. It was 22.2% in men in the obese group. It was 16.7% in women in the obese group. But the difference between the two groups was not statistically significant(χ2=0.278, P>0.05).3 The OB group was allocated three groups by BMI, and the results of serum ADPN:the first group and the third group, the first group and Con group, the second group and Con group, the three group and Con group, the difference was statistically significant(4.03±1.26 vs 2.86±1.20,4.03±1.26 vs 4.95±0.25,3.63±1.90 vs 4.95±0.25, 2.86±1.20 vs 4.95±0.25,P<0.05). The results of urine ALB:the first group and the third group, the first group and Con group, the second group and Con group, the three group and Con group, the difference was statistically significant(12.50±6.15 vs 33.85±29.85,12.50±6.15 vs 7.26±6.48,23.36±20.51 vs 7.26±6.48,33.85±29.85 vs 7.26±6.48, P<0.05).4 The OB group was allocated three groups by the level of serum ADPN,the urine ALB will gradually increase with the decrease of the aerum ADPN, the difference in four group was statistically significant(Con组:7.26±6.48, A组: 12.97±5.64, B组:15.73±15.43, C组:46.83±27.86). 5 The OB group was allocated two groups by sex, and the results:The level of serum ADPN in men in OB group was lower notably than that in women in OB group. The difference was statistically significant(3.14±1.60 vs 4.08±1.25, P<0.05).6 The results of linear correlation analysis revealed that the level of serum ADPN in OB group was in a negative correlation with BMI, WC, LEP, TG, TC, BUN, ACR, and there was statistically significant(r=-0.397,-0.345,-0.313,-0.406,-0.393,-0.002,-0.370, P<0.01). The level of ADPN in OB group was in a negative correlation with WHR, MAP, Glu, and there was statistically significant (r=-0.181,-0.476,-0.300, P<0.05). The level of serum ADPN in OB group was in a negative correlation with LDL-c, CRE, UA, GFR, and there was not statistically significant(r=-0.335,-0.126,-0.066,-0.077, P>0.05). The level of ADPN in OB group was in a positive correlation with HDL-c, and there was not statistically significant(r=0.265,P<0.05). The level of urine ALB in OB group was in a positive correlation with BMI, and there was statistically significant(r=0.329, P<0.01). The level of urine ALB in OB group was in a positive correlation with WC, and there was statistically significant(r=0.0.283, P<0.01). The level of urine ALB in OB group was in a positive correlation with MAP, TG, TC, and there was statistically significant(r=0.129,0.276,0.210, P<0.05). The level of urine ALB in OB group was in a positive correlation with Glu, HDL-c, LDL-c, BUN, CRE, LEP, and there was not statistically significant(r=0.108,0.118,0.053,0.025,0.242,0.078, P>0.05). The level of urine ALB in OB group was in a negative correlation with UA, and there was not statistically significant(r=-0.101,P>0.05).Conelusions:The level of serum ADPN and urine ACR are related to obesity. The level of serum ADPN will decrease with the increase of BMI in obesity patients,while the level of urine ACR will increase with the increase of BMI in obesity patients. There is a negative correlation between serum ADPN and urine ACR. This infers that the decrease of serum ADPN maybe one of reasons for urine ALB in obesity patients.This can offer the clinical evidence for serum ADPN as a protectin for the kidney in obesity patients. The level of serum ADPN in men with obesity is lower than that in women with obesity,at the same time, The level of urine ACR is higher than that in women with obesity.This infers the kidney of men with obesity will be vulnerable. The detection of serum ADPN maybe a index of early diagnosis of ORG,and offer a new target for the treatment of ORG. |
