Purpose:To study the correlation between ultrasonographic features and the pathohistology types and grades and the expression of molecular biological markers ER, PR and C-erbB-2 of breast cancer. Analyze the pathological bases of ultrasonogram and the effect of tumor biological characteristics on ultrasonographic features of breast cancer, in order to improve the level of ultrasound imaging technology in diagnosis of breast cancer and provide reliable imaging information for tumor clinical treatment options and prognosis, Thus further promote ultrasound examination in the value of clinical diagnosis of breast cancer.Methods:Collected 137 cases of breast malignant solid lesions, which underwent surgical and confirmed by pathology in our hospital from November 2008 to October 2010, Age from 26 to 81 years. All passed preoperative ultrasound, no radiotherapy and chemotherapy treatment. The ultrasonographic features and paraffin section of 137 cases of breast cancer confirmed by histopathology were observed retrospectively. Ultrasound indicators included tumor size, shape, ratio of the longest/shortest dimension, edge of the spiculation and hyperechoic halo sign, internal echo types, microcalcification, posterior echo types, blood flow and signs of lymph node metastasis. Pathological indicators included pathological types of breast cancer and histological grade of infitrating ductal carcinoma(IDC), composition ratio of cancer cell and interstitial of the lesion's edge and interior under the microscope field of vision and the expression of molecular markers ER, PR and C-erbB-2, to investigate the correlation between sonographic features and the pathology of breast cancer; Comparatively studied the tissue composition characteristics of the lesion's edge and interior respetively according to the pathology and ultrasound; To investigate the correlation between the ultrasonographic features with pathology and molecular biology (ER,PR,C-erbB-2) of breast cancer. All data processed software by SPSS 13.0, Using x2 test,P<0.05 was considered statistically significant.Results:①IDC were lager (>2cm)than ductal carcinoma in situ (DCIS) and early IDC, there were significant differences (P<0.05), but did not significantly correlated with particular types of breast cancer (P>0.05); The rate of irregular mass was higher in the IDC than in the particular types of cancer (P<0.05), there was no significant difference with DCIS and early IDC(P>0.05); The frequency of spiculate margin was higher in the IDC than in the DCIS, early IDC and particular types of cancer (P<0.05); Tumor ratio of the longest/shortest dimension, hyperechoic halo sign, internal echo types, microcalcification, posterior echo types were not significantly correlated with the types of breast cancer (P>0.05). The rate of irregular mass was higher in the gradeⅢIDC than in the gradeⅠand gradeⅡIDC (P>0.05).The rate of acoustic shadowing mass was higher in the gradeⅠIDC than in the gradeⅡand gradeⅢIDC (P>0.05). Tumor size, edge of the spiculation and hyperechoic halo, internal echo types and microcalcification were not significantly correlated with histological grade of breast cancer (P>0.05). (2)Tumors with larger volume (> 2cm), internal hypoechogenicity and posterior echo enhancement had higher tissue grade, high proportion of cancer cell (P<0.05). Tumors with hyperechoic halo sign had higher tissue grade, high proportion of cancer cell (P<0.05). The shape, ratio of the longest/shortest dimension and spiculate margin of breast cancer were not significantly correlated with the composition of the internal components of tumor tissue(P>0.05). (3)Tumor ratio of the longest/ shortest dimension>1, spiculate margin and hyperechoic halo sign of breast cancer were correlated with the positive expression of ER and PR(P<0.05); Tumor internal necrosis was correlated with the negative expression of PR(P<0.05); The microcalcification was correlated with the positive expression of C-erbB-2(P<0.05). Tumor size, shape, posterior echo types, lymph node metastasis and blood flow had no significant correlation with the expression of ER, PR and C-erbB-2 (P>0.05).Conclusion:(1) Ultrasonogram of breast cancer had its corresponding pathological bases. Ultrasonographic features were related to pathohistological types and grades within a certain range, may indicate histopathological types and grades. (2) There were some correlations between breast cancer ultrasonographic features and molecular biology markers. Tumor biological characteristics influenced breast cancer ultrasonogram. Ultrasonography can provide imaging bases for breast cancer prognosis assessment and clinical treatment.
