Objective To analyze influencing factors and treatment of cerebrospinal fluid l eakage in the sellar tumour resection through Endoscopic endonasal transsphenoidal approach. Methods The clinical records of 42 patients, who had cerebrospinal fl uid leakage in 246 cases of endoscopic endonasal transsphenoidal approach, were r eviewed. to analyze the causes of cerebrospinal fluid leakage, it's relation to the tu mor size, nature, location and type. Results sellar tumour resection through transs phcnoidal approach may happen cerebrospinal fluid leakage, but carefull reconstructi on on operation will get successful results.there is no cerebrospinal fluid rhinorrhea in 41 patients exepet one dead. Conclusions there is a rare reports about the ca uses and incidence of cerebrospinal fluid leakage in the sellar tumour resection thr ough transsphcnoidal approach, it's related to the experience of the surgeon, the t umor size, nature, location and type. carefull reconstruction using autologous tissue plus artificial materials, postoperative lying in bed, lumber drainage on necessary will effectively prevent postoperative cerebrospinal fluid rhinorrhea.
