Objective To study the clinical experience of the microneurosurgical operation in the sellar region meningiomas via interhemispheric approach.Methods Clinical data of 32 cases with sellar meningiomas were retrospectively analyzed.Sellar region meningiomas which had been operated through the interhemispheric approach in microneurosurgical technique in the 1st hospital of the Shanxi medical university during Jan 2000 to Dec 2006. We discussed the factors that can affect the choice of operation approach. The operative results and complications were discussed with microneurosurgical techniques.Results Thirty two patients had obtained good results of all cases, the sellar region meningiomas total resection SimpsonII(78.1%),SimpsonIII(22.5%).The preoperative visual acuity and visual field pathologyical change ,extent of tumour removal have some influence on the postoperative visual recovery.Eight patients were diabetes.The follow-up survery for 2.5 years.There have no meningiomas recurrence.No patient died after the surgery .Conclusion The interhemispheric approach is the satisfactory one in most cases. It can achieve sufficient structure exposure in lateral tumor , pathological of sellae meningiomas and suprasellar.In order to reduce the post operative complications,anterior cerebral artery , optic chiasma and pituitary stalk must be protected during the interhemispheric approach operation.It is concluded that satisfactory outcome could be expected when the correct microneurosurgical operation approach used on patient with larger sellae meningiomas.
