OBJECTIVE:To investigate the expression of melatonin in follicular fluid in women with and without polycystic ovary syndrome (PCOS). To examine the relationships between melatonin concentration in follicular fluid,oxidative stress and clinical outcome in PCOS patients undergoing assisted reproduction treatment.METHODS:40 patients with PCOS and 22 patients of control group were selected from Reproductive Centre of Xiangya Hosptial from September to December 2010. Follicular fluids of these patients were collected at ovum pick-up day. After centrifugation, the total antioxidant capacities (TAC) were detected by sub-visible sepctrophotometric method, melondialdehyde (MDA) detected by thiobarbituric acid (TBA) method, while melatonin (MET) detected by euzymelinked immunosorbent assay (ELISA). Compare the differences of MET, TAC, MDA levels, and clinical outcome of the two groups to verify whether there are any statistical significance.RESULTS:1. Compared with the control group, the average dose of Gn used during controlled ovarian stimulation were lower in PCOS patients (1.71±0.56ampouleVS.2.02±0.55ampoule), and the oocyte mature rate (72.3% VS.86.7%), normal fertility rate(73.4% VS.80.1%) and utilization rate of oocyte(66.4% VS.74.8%) and utilization rate of embryos(72.4% VS.89.9%) in PCOS group were lower(P<0.05), but there was no significant difference in high-quality embryo rate(66.3%VS.72.1%), implantation rate(34.7%VS.39.5%), clinical pregnancy rate (57.1%VS. 65.0%) and abortion rate(5%VS.0%) between the two groups(P>0.05).2.The levels of MET(113.63±27.89pg/ml VS.135.99±28.35pg/ml) and TAC (16.32±2.60IU/ml VS.18.09±3.56IU/ml) in follicular fluid were lower in women with PCOS than that of control group, but the MDA levels (2.25±0.41nmol/mlVS.1.91±0.44nmol/ml) were higher in women with PCOS(P<0.05).3. The correlation analysis showed that MET and TAC in follicular fluid is positively correlated(r=0.694, P=0.000).CONCLUSION:1.There were increased sensitivity to Gn and lower oocyte mature rate, fertility rate, utilization rate of oocyte and embryo in PCOS patients, which indicated the oocyte of PCOS may be declined in quality.2.There was lower level of melatonin in follicular fluid of patients with PCOS, as MET being positive relative to the total antioxidant capacity, MET may be involved in the oocyte oxidation injury.
