ObjectiveComparative the effect and complications between One– stage and two– stage combined anterior - posterior approach on pinching cervical spondylotic myelopathy in order to provide some valuable reference about the treatment of cervical spondylotic myelopathy.MethodThis subject analyzes 46 CSM cases that undertook the operation of anterior - posterior approach and follow-up in the orthopedic department of the zhujiang Hospital of Southern Medical University and the orthopedic department of the Affiliated Hospital of traditional chinese medicine university of Liaoning from December 2007 to May 2009. Cohorts were divided into two groups: One– stage group,29 cases who undertook One– stage combined posterior- anterior approach ;two– stage group,17 cases who received two– stage combined posterior- anterior approach. The gender ,age ,course of disease Imaging Findings and JOA scores of cases in the two groups is no no marked difference in statistical analysis.to assess the JOA scores,Cervical sagittal alignment(D numerical value) and the theperioperative complications of the cases in two groups of one week and six months after operation. ResultJOA scores:The statistical difference between two groups didn't exist before surgery(P=0.465).A great improvement was made after operation each group(P=0.0001), excellent rate in One– stage group is higher than that in two– stage group of the first and the second operation,and excellent rate of the first operation is higher than that of the second operation.The improvement rate both in two groups of 6 months after operation went up than that of one week after operation(P=0.046), excellent rate in One– stage group is higher than that in two– stage group 6 months after surgery(P=0.041).The two groups got 100% impovement rate after surgery.Perioperative complications:The incidence of two groups was 17.65 percent and 27.59 percent respectively.There was no severe complication such as bone graft non fusion,looseness of the plate fixation,reclose opening-door etc. Cervical sagittal alignment(D numerical value):There was no statistical difference at the preoperative between two groups(P=0.435). the difference between the postoperative and the preoperative was stastically significant(P=0.0001) in each groups,but there was no remarked differece between two groups (P=0.813)and no change after 6 months follow-up.Conclusion1.Sergical treatment has a positive urative effect on CSM2. one– stage combined anterior - posterior approach on pinching cervical spondylotic myelopathy is obviously better than two– stage combined anterior - posterior approach, One– stage combined posterior- anterior approach can provide better neurological recovery than two-stage approach in treatment of pinching cervical spondylotic myelopathy.3.One-stage is involved in more perioperation complications...
