Objective:To explore the composition and drug-sensitivity of the pathogens of hospital-acquired pneumonia in patients with strok.Methods:Of January 2004-December 2008,The First Affiliated Hospital of Shanxi Medical University,Department of Neurology,treated 90 cases of acute stroke combined hospital-acquired pneumonia patients.Their respiratory secretions training results were analyzed retrospectively.Results:90 cases of acute stroke merger hospital-acquired pneumonia patients with respiratory secretions training result isolated strains of pathogenic 170 strains.There were 55 cases of two or more pathogen infection,and there were 35 patients of one pathogen infection.Of 170 clinical strains isolated,Gram negative organisms accounted for 101(59.41%).Its distribution were pneumonia crayresearch bacteria(27 strains,26.73%),a.baumanii(13 strains,12.87%), escherichia coli(10 strains,9.90%),p.aeruginosa(10 strains,9.90%),bloodsucking bacili(9 strains 8.91%),etc.Gram positive organisms accounted for 49(28.82%).The main isolated strains were Coagulase-negative staphylococci and Staphyloccoccas aureus.Ampicillin.Fungi accounted for 20(11.76%).They were main Candida albicans.There were 4 ESBLs(+) strains separated from 10 escherichia coli strains,the separated rate was 40%.27 strains of bacteria pneumonia crayresearch contained 5 ESBLs(+) strains(18.52%).The total separation ratio was 24.32%.30 plants coagulase-negative staphylococci included resistant strains of quality,which MRCNS(22),separated rate was 73.33%.5 strains of staphylococcus ball contained resistant strains of a oxygen cylindriod(MRSA) 4,the separation ratio is 80%.The drug susceptibility carbapenems results showed that:Carbapenem antibiotics (Meropenem and Imipenem) showed excellent antibacterial activity(75%-100%) to gram-negative bacteria.Gram-negative bacteria had low sensitivity to ceftriaxone,cefoperazone. Most of the gram-negative bacilli of nosocomial infection inhibitors increase the sensitivity of antibiotics than not higher,particularly cefoperazone cefoperazone sulbactam compared with more obvious difference There was significant.Drugs tested by Pseudomonas aeruginosa is the most sensitive to imipenem,and piperacillin / tazobactam and imipenem-sensitive rate of less than cefoperazone / sulbactam.This group gram-positive bacteria were found in vancomycin-resistant strains,suggested that gram-positive bacteria sensitive to vancomycin.Besides,coagulase-negative staphylococci, Streptococcus pneumoniae,Staphylococcus aureus had higher sensitivity to linezolid,teicoplanin, but also showed that the triazole flavanone with higher sensitivity of antibiotics.Research data show that the gram-positive bacteria were more sensitive to levofloxacin, fluoroquinolones,and that can be seen on the treatment of gram-positive bacteria was still very effective.Conclusion:The combined acute stroke hospital-acquired pneumonia pathogens mainly to gram-negative bacteria,and pathogens for the conditions.Pathogens have had varying degrees of resistance to commonly used antibiotics like penicillin,ampicillin.And gram-negative bacteria were more sensitive to Carbapenem antibiotics,β-lactamase complex agents.And found no vancomycin resistant Gram-positive cocci.
