Objective: To study the application prospect of treating the middle and late stage prostatic carcinoma using 125I radion implantation by transrectal ultrasound combined with endocrinotherapy.Method :Review 27 cases of middle and late stage prostatic carcinoma, of which 7 are in Stage C and 20 are in Stage D. 13 of them were given operation castration or medicine castration combined with antiandrogen , and implanted 125I by transrectal ultrasound ; 14 were given operation castration or medicine castration combined with antiandrogen. The conditions before the treatment of both groups were compared, and Sas 8. 2 was used to analyze the statistics, and measurement data of the samples were compared, e. g. t test was used when the mean square was regular, while rank sum test was used when the mean square was not regular; when compared the enumeration data, e.g. x2 test was used when the mean square was regular, while CMH test was used when the mean square was not regular.Result: 13 cases of the treatment group was with a successful operation.66 particles were implanted on average, nd the mean operation time were 2.5-3 hours, nd ureter was detained for 1-4 weeks after the operation. 1-48 months of follow-up visit was done, and the average time was 20 months.(1) Prostate volume: The volumes of prostates were obvious diminished in the 6th month of the treatment when comparing the treatment group with before the treatment and with the control group. The difference was statistical significant with P<0. 01.(2)KPSS score: When treated for 3 months, compared the treatment group with before the treatment and with the control group, P<0. 05, and the score of KPSS was increased with a statistical significance; when treated for 6 months, P<0. 01 and the difference was significant. (3)PSA: Compared the two groups in the 1st and 3rd month of the treatment, P<0. 05 and the difference were significant. In the 6th and 12th month, P<0.01 and the difference was significant. The treatment group was obviously better than the control group.(4)According to the objective reactive standard of Spain the 1st month of the treatment, the two groups were compared with P>0. 05 and the difference was not statistically significant while in the 3rd month, P<0. 05 and the difference was statistically significant. The treatment group was better. In the 6th and 12th month of the treatment, P<0. 01 and the difference was obvious .The treatment group was obviously better than the control group.(5)Survival Rate: There was no difference in 1-3 years of survival rate.(6)Complication: Of the 13 cases,11 had complications in the urinary system after the operation .All of the complications were LevelⅠ~Ⅱand were relieved after the detained ureter. 1 case had uroclepsia after 12 months and had the detained ureter till now. 3 had intestinal complications and of which 2 were Level I and 1 were LevelⅢ.Conclusion:125I radion implantation combined with endocrinotherapy is a safe and effective treatment compared with mere endocrinotherapy when treating the middle and late stage prostatic carcinoma and was worth generalizing.
